CT聯(lián)合超聲鑒別腮腺良惡性腫瘤的價(jià)值

【摘要】:目的:探討CT檢查聯(lián)合超聲檢查在診斷和鑒別診斷腮腺良惡性腫瘤中的應(yīng)用價(jià)值,以提高術(shù)前診斷率,為臨床制定治療方案提供參考依據(jù)。資料與方法:回顧性分析我院2008年2月-2012年11月收住的117例病例資料完整的腮腺腫瘤患者術(shù)前CT和超聲檢查的影像學(xué)資料,CT上分析腫瘤位置、大小、形態(tài)、邊界、密度、強(qiáng)化表現(xiàn)、是否侵犯周?chē)M織結(jié)構(gòu)以及有無(wú)淋巴結(jié)轉(zhuǎn)移等情況;超聲檢查將二維超聲和彩色多普勒顯像(CDFI)相結(jié)合,觀察病灶的位置、大小、形態(tài)、邊界、內(nèi)部回聲、后方回聲、血流分布及強(qiáng)度,根據(jù)CT成像或超聲成像分別進(jìn)行診斷,并綜合CT及超聲表現(xiàn)進(jìn)行聯(lián)合診斷,分別與病理診斷結(jié)果進(jìn)行比較,作相關(guān)統(tǒng)計(jì)學(xué)分析。結(jié)果:CT對(duì)病變的定位符合率達(dá)到94.87%;CT成像對(duì)117例腮腺腫瘤的診斷符合率為77.78%,超聲對(duì)117例腮腺腫瘤的診斷符合率為78.67%,兩者之間的差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);CT與超聲聯(lián)合應(yīng)用的診斷符合率達(dá)89.74%(105/117),與單獨(dú)應(yīng)用CT成像或超聲相比,之間的差異均有統(tǒng)計(jì)學(xué)意義(P0.05):兩者聯(lián)合應(yīng)用于腮腺腫瘤診斷的準(zhǔn)確性明顯優(yōu)于二者獨(dú)立應(yīng)用。結(jié)論:腮腺良、惡性腫瘤,在部位、形態(tài)、邊界、侵襲性、淋巴結(jié)轉(zhuǎn)移等征象上有顯著差異,CT與超聲檢查對(duì)腮腺良、惡性腫瘤的診斷及鑒別診斷均有較高的應(yīng)用價(jià)值,CT和超聲聯(lián)合應(yīng)用于腮腺腫瘤的診斷,可提高術(shù)前影像學(xué)對(duì)腮腺良惡性腫瘤診斷的符合率。
[Abstract]:Objective: to explore the value of CT combined with ultrasound in the diagnosis and differential diagnosis of benign and malignant tumors of parotid gland in order to improve the preoperative diagnosis rate and provide reference for clinical treatment. Materials and methods: Ct and ultrasonographic data of 117 patients with parotid gland neoplasms admitted in our hospital from February 2008 to November 2012 were analyzed retrospectively. The location, size, shape, boundary and density of the tumors were analyzed on CT. Enhanced findings, invasion of surrounding tissue structure and lymph node metastasis, ultrasound examination combined two-dimensional ultrasound with color Doppler imaging (CDFI) to observe the location, size, shape, boundary, internal echo, rear echo, and so on. The distribution and intensity of blood flow were diagnosed according to CT imaging or ultrasound imaging, and combined with CT and ultrasound findings. The results were compared with pathological diagnosis, and the correlation statistical analysis was made. Results the diagnostic coincidence rate between CT and ultrasound was 77.78 for 117 parotid gland tumors and 78.67 for ultrasound in 117 parotid gland tumors. There was no significant difference between them. The diagnostic coincidence rate was 89.7475 / 117, compared with CT imaging or ultrasound alone. The difference between the two groups was statistically significant (P 0.05): the accuracy of the combined application in the diagnosis of parotid gland tumors was significantly better than that of the two independent applications. Conclusion: there are significant differences between benign parotid gland, malignant tumor, location, shape, boundary, invasion, lymph node metastasis and so on. The diagnosis and differential diagnosis of malignant tumors have higher application value. Ct and ultrasound can be used in the diagnosis of parotid gland tumors, which can improve the coincidence rate of preoperative imaging in the diagnosis of benign and malignant tumors of parotid gland.
