【摘要】：目的探討CT能譜曲線鑒別診斷脊柱惡性腫瘤與非惡性腫瘤及腫瘤樣病變的價(jià)值。方法對(duì)38例經(jīng)手術(shù)或穿刺病理證實(shí)的脊柱腫瘤及腫瘤樣病變患者行能譜CT掃描,包括非惡性組18例,其中良性和中間型腫瘤及腫瘤樣病變分別為12例和6例;惡性組20例。獲得單能量圖像,生成CT能譜曲線,計(jì)算40~140keV不同能級(jí)水平下的CT值和能譜曲線斜率。同時(shí)獲得病變?cè)诨旌夏芰繄D像的靜脈期CT值。對(duì)所得CT值和能譜曲線斜率進(jìn)行統(tǒng)計(jì)學(xué)分析。結(jié)果 40~80keV非惡性組CT值分別為(197.97±95.69)HU、(141.30±63.66)HU、(105.78±44.04)HU、(84.21±32.65)HU、(68.92±26.16)HU,惡性組CT值分別為(321.86±97.97)HU、(225.46±72.42)HU、(159.98±50.03)HU、(120.53±41.69)HU、(96.59±37.86)HU。40~80keV條件下,非惡性組與惡性組相同能級(jí)CT值差異均有統(tǒng)計(jì)學(xué)意義(P均0.05);90~140keV條件下,非惡性組與惡性組相同能級(jí)CT值差異均無(wú)統(tǒng)計(jì)學(xué)意義(P均0.05)。非惡性組能譜曲線斜率為1.58±0.89,低于惡性組能譜曲線斜率(2.76±0.83;t=-3.48,P=0.002)。非惡性組與惡性組混合能量圖像的CT值差異無(wú)統(tǒng)計(jì)學(xué)意義(t=-1.19,P=0.250)。結(jié)論 CT能譜曲線對(duì)鑒別惡性與非惡性脊柱腫瘤及腫瘤樣病變有一定價(jià)值,具有較好的應(yīng)用前景。
[Abstract]:Objective to study the value of CT energy spectrum in differentiating spinal malignant tumor from non malignant tumor and tumor-like lesion. Methods EDS CT scans were performed in 38 patients with spinal tumors and tumor-like lesions confirmed by operation or puncture pathology, including 18 cases of non-malignant group, including 12 cases of benign and 6 cases of benign and intermediate tumors and 6 cases of tumor-like lesions, and 20 cases of malignant group. The single energy image is obtained and the CT energy spectrum curve is generated. The CT value and the slope of the energy spectrum curve are calculated at different energy levels of 40~140keV. At the same time, the CT values of the lesions in the mixed energy images were obtained.
