寶石CT低劑量評(píng)價(jià)肺功能的可行性初探

【摘要】:目的: 探討寶石能譜CT低照射劑量評(píng)價(jià)肺功能的可行性。 材料與方法: 隨機(jī)抽取2014年1月1日至2014年3月20于我院體檢人員60名,分為正常照射劑量組(NI=11)及低照射劑量組(NI=18)。正常照射劑量組受檢者30例,其中男20例,女10例,年齡范圍20~61歲,平均年齡40.8±10.8歲;低照射劑量組(NI=18)受檢者30例,其中男性16例,女性14例,年齡范圍24~62歲,平均年齡為44.8±11.1歲。所有受檢者行寶石能譜CT胸部吸氣末屏氣掃描,利用后處理軟件ThoracicVCAR分析圖像數(shù)據(jù)并測(cè)量右肺上葉尖段氣道壁平均厚度(AverageWall Thickness,WTAvg)、管徑厚度與外徑比值(Wall Thickness/AirwayDiameter,WT/D)、氣道壁面積與氣道總面積比值(Wall AreaRatio,WAR)、氣道壁面積(Wall area,WA);測(cè)量出雙肺各CT值區(qū)域(分為-1024~-910Hu,-910~-800Hu,-800~-700Hu,-700Hu)肺像素指數(shù)(pixel index,,PI),評(píng)估兩組各測(cè)量參數(shù)間有無(wú)統(tǒng)計(jì)學(xué)差異。記錄兩組受檢者CT容積劑量指數(shù)(CTDIvol)、劑量長(zhǎng)度乘積(DLP)并計(jì)算有效劑量(ED)。 結(jié)果: 利用spss19.0軟件分析實(shí)驗(yàn)數(shù)據(jù),經(jīng)配對(duì)T檢驗(yàn)后得出:兩組受檢者右肺上葉尖段氣道參數(shù)WTAvg、WT/D、WAR、WA間p值分別為0.065、0.559、0.585、0.131;兩組受檢者雙肺各區(qū)域像素指數(shù)PI-1024~-910Hu、PI-910~-800Hu、PI-800~-700Hu、PI-700Hu間P值分別為0.262、0.817、0.542、0.889。兩組各項(xiàng)數(shù)據(jù)間均無(wú)統(tǒng)計(jì)學(xué)差異(p0.05)。正常劑量組、低劑量組ED分別為4.99±1.37、1.92±0.68,低劑量組ED較正常劑量組降低61.52%。 結(jié)論: 寶石CT低照射劑量(NI=18)評(píng)價(jià)肺功能可行。
[Abstract]:Objective: Objective: to evaluate the feasibility of low dose gemstone CT in evaluating pulmonary function. Materials and methods: From January 1, 2014 to March 20, 2014, 60 medical examiners in our hospital were randomly divided into normal irradiation dose group and low dose group. 30 patients (male 20, female 10, mean age 40.8 鹵10.8) in the normal radiation dose group, and the low dose group (16 males and 14 females, with an average age of 44.8 鹵11.1 years). All subjects were scanned by gemstone energy dispersive computed tomography (EDS) at the end of inspiratory breath holding. Using post-processing software ThoracicVCAR to analyze the image data and to measure the mean thickness of the airway wall in the upper apical segment of the right lung (AverageWall Thicknessy WTAvgn), the ratio of diameter thickness to outer diameter, Wall thickness to outer diameter ratio, Wall area to total area of the airway, Wall area of the upper lobe of the right lung, and the area of the airway wall to measure the area of the two lungs, and to measure the ratio of the wall area to the total area of the airway. The pixel index of each CT value region (divided into -1024 ~ 910Hu-910Hu-800 Hu-800 Hu-800 Hu-700 Hu-700 Hu-700 Hu-7H) lung pixel index was used to evaluate the statistical difference between the two groups. The CT volumetric dose index (CTDIvoll), dose length product (DLPP) and effective dose (EDV) were recorded in the two groups. Results: Spss19.0 software was used to analyze the experimental data. After paired T test, the following results were obtained: the airway parameters of the right upper pulmonary apical segment in the two groups were as follows: the value of the WTAvgGV WTP / WARWA was 0.065 0. 559 0. 585% 0.131, and the pixel index of each region of the lung was PI-1024-910Hu-910Hu-800 Hu-PI-800 Hu-700 Hu value of 0.2620.817U 0.5420.80.88898.The two groups were divided into two groups, and the results showed that the mean values of the two groups were 0.2620.817 and 0.5420.8889, respectively, and the pixel index of the two groups was PI-1024-910Hu-910Hu-800 Hu-PI-800, PI-700Hu. There was no statistical difference between the two groups (p 0.05). Ed in normal dose group and low dose group were 4.99 鹵1.37 and 1.92 鹵0.68 respectively. Ed in low dose group was 61.5222lower than that in normal dose group. Conclusion: The evaluation of lung function by low dose sapphire CT was feasible.
