【摘要】：目的:總結分析艾滋病相關腹部淋巴瘤的CT表現(xiàn)特征,對比非艾滋病與艾滋病相關淋巴瘤(ARL)的臨床及影像學表現(xiàn)異同。方法:回顧性分析28例經手術病理證實的艾滋病相關淋巴瘤患者的臨床及CT表現(xiàn)。結果:28例艾滋病相關淋巴瘤中霍奇金病1例,非霍奇金淋巴瘤27例,受累臟器包括肝臟10例、脾臟9例、胰腺6例、腎臟5例、胃腸道8例、結內病變23例。病變器官解剖結構留存,病灶多數密度均勻,呈輕中度均勻強化,小部分病變中央出現(xiàn)壞死,呈環(huán)形輕中度強化。淋巴結以腹主動脈周圍、腹腔干周圍、腸系膜、腸系膜根部最為好發(fā)。ARL病灶侵襲性高,累及范圍廣,對放化療反應性較差。結論:ARL的CT表現(xiàn)具有一定特征性,結合臨床表現(xiàn),有利于提高診斷符合率。
[Abstract]:Objective: to summarize the CT features of AIDS-associated abdominal lymphoma and compare the clinical and imaging features of AIDS-associated lymphoma with AIDS-associated lymphoma. Methods: the clinical and CT findings of 28 patients with AIDS-associated lymphoma confirmed by operation and pathology were retrospectively analyzed. Results among 28 cases of AIDS-related lymphoma, one case was Hodgkin's disease and 27 cases were non-Hodgkin 's lymphoma. The involved organs included liver (10 cases), spleen (9 cases), pancreas (6 cases), kidney (5 cases), gastrointestinal tract (8 cases), and nodal lesion (23 cases). The anatomical structure of the diseased organs remained, the density of most lesions was even, the lesions showed mild to moderate homogeneous enhancement, a small number of lesions showed necrosis in the center, and the lesions were ring mildly and moderately enhanced. The lymph nodes around the abdominal aorta, around the celiac trunk, mesentery and mesentery root were the most aggressive and involved, and the reactivity to radiotherapy and chemotherapy was poor. Conclusion the CT findings of 10% ARL have some characteristics, and combined with clinical manifestations, it is helpful to improve the diagnostic coincidence rate.
