Revolution CT軸位全肝灌注“一站式”成像的可行性

【摘要】:目的探討Revolution CT軸位掃描全肝灌注"一站式"成像的可行性。方法選取19例患者行Revolution CT上腹部增強(qiáng)檢查,獲得全肝CT灌注(CTP)圖像、靜脈期及平衡期圖像。于CTP圖像上由2名觀察者分別確定腹主動(dòng)脈與門靜脈主干時(shí)間-密度曲線(TDC)峰值對應(yīng)的rank值及CT值,定量測量肝左、右葉灌注參數(shù)血流量(BF)、血容量(BV)、平均通過時(shí)間(MTT)、達(dá)峰時(shí)間(TP)、肝動(dòng)脈分?jǐn)?shù)(HAF)。采用腹主動(dòng)脈和門靜脈TDC峰值所對應(yīng)的圖像重組肝動(dòng)脈CTA、門靜脈CTV,并提取動(dòng)脈期圖像。記錄CT灌注和"一站式"檢查的輻射劑量。比較肝左、右葉灌注參數(shù)的差異,并對2名觀察者的結(jié)果進(jìn)行一致性分析。結(jié)果肝左葉、右葉的BF和MTT值差異有統(tǒng)計(jì)學(xué)意義(P均0.05);2名觀察者對肝動(dòng)脈CTA、門靜脈CTV及動(dòng)脈期圖像的主觀評分均≥2分,一致性良好(Kappa值均0.6)。灌注期和"一站式"檢查有效輻射劑量分別為14.47mSv、21.29mSv。結(jié)論 Revolution CT軸位全肝灌注"一站式"成像,在保證較低輻射劑量的前提下,可獲得肝臟CTP的多個(gè)定量參數(shù),又可提供清晰的肝動(dòng)脈CTA、門靜脈CTV和3期增強(qiáng)掃描圖像,具有廣闊的臨床應(yīng)用前景。
[Abstract]:Objective to investigate the feasibility of Revolution CT axial-scan for one-stop imaging of whole liver perfusion. Methods Nineteen patients were examined with Revolution CT upper abdominal enhancement, and the CT perfusion images, venous phase and balance phase images were obtained. The rank and CT values corresponding to the peak value of time-density curve of abdominal aorta and portal vein were determined by two observers on CTP images, and the left liver was measured quantitatively. Right lobe perfusion parameters: BFV, BV, mean transit time (MTT), peak time (TTP), hepatic artery fraction (HAF). The images corresponding to the peak of TDC in abdominal aorta and portal vein were used to recombine hepatic artery and portal vein, and the arterial phase images were extracted. The radiation doses of CT perfusion and one stop examination were recorded. The difference of perfusion parameters between left and right lobes of liver was compared and the results of two observers were analyzed. Results the difference of Zuo Ye, BF and MTT in right lobe was statistically significant (P < 0.05).
