數(shù)字乳腺斷層融合X線成像中平均腺體劑量與乳腺密度、壓迫厚度的關(guān)系

【摘要】:目的:探討數(shù)字乳腺斷層融合X線成像(DBT)中平均腺體劑量(AGD)與乳腺密度、壓迫厚度的關(guān)系。方法:搜集在DBT Combo模式[即DBT和全視野數(shù)字化乳腺攝影(FFDM)分別在同一乳腺、在同一壓迫厚度下進(jìn)行攝影]下行雙側(cè)乳腺攝影的女性患者271例。分析共542幅DBT和542幅FFDM的單側(cè)乳腺頭尾位圖像。結(jié)果:(1)壓迫厚度與AGD_(DBT)呈正向較強(qiáng)相關(guān)(r=0.920,P0.001),乳腺密度與AGD_(DBT)呈正向較弱相關(guān)(r=0.263,P0.001)。壓迫厚度和乳腺密度分別是AGD_(DBT)的獨(dú)立影響因素(B_(壓迫厚度)=0.046,B_(乳腺密度)=0.084,P0.001)。壓迫厚度對AGD_(DBT)的影響較乳腺密度大(壓迫厚度、乳腺密度的標(biāo)準(zhǔn)化系數(shù)分別為0.940、0.143)。(2)壓迫厚度與△AGD(AGD_(DBT)減去AGD_(FFDM))呈負(fù)向較弱相關(guān)(r=-0.287,P0.001),乳腺密度與△AGD呈負(fù)向中等相關(guān)(r=-0.616,P0.001);乳腺密度和壓迫厚度分別是△AGD的獨(dú)立影響因素(B_(壓迫厚度)=-0.012,B_(乳腺密度)=-0.219,P0.001);乳腺密度對△AGD的影響較壓迫厚度大(壓迫厚度、乳腺密度的標(biāo)準(zhǔn)化系數(shù)分別為-0.580、-0.366)。(3)在乳腺密度75%且壓迫厚度為56~75mm和76~95mm時,AGD_(DBT)均值低于AGD_(FFDM),差異有統(tǒng)計(jì)學(xué)意義(P0.05)。在乳腺密度為25%~50%且壓迫厚度為76~95mm時,AGD_(DBT)均值與AGD_(FFDM)相仿(P0.05)。在乳腺密度為51%~75%且壓迫厚度為56~75mm時,AGD_(DBT)均值與AGD_(FFDM)相仿(P0.05)。在其他乳腺密度和壓迫厚度分層中(壓迫厚度均≤95mm),AGD_(DBT)均值高于AGD_(FFDM),差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:AGD_(DBT)與乳腺密度、壓迫厚度呈正相關(guān),壓迫厚度對AGD_(DBT)的影響更大!鰽GD與乳腺密度、壓迫厚度呈負(fù)相關(guān);乳腺密度對△AGD的影響更大。在乳腺密度75%且壓迫厚度為56~95mm時,AGD_(DBT)均值低于AGD_(FFDM);在其他乳腺密度及壓迫厚度分層(壓迫厚度均≤95mm)中,AGD_(DBT)均值高于或等于AGD_(FFDM)。
[Abstract]:Objective: to investigate the relationship between the average glandular dose (AGD) and breast density and compression thickness in digital mammographic fusion X ray imaging (DBT). Methods: a total of 271 female patients underwent bilateral mammography under DBT Combo mode (DBT and full field digital mammography FFDM) under the same mammary gland and under the same compression thickness respectively. A total of 542 DBT and 542 FFDM images of unilateral breast head and tail were analyzed. Results (1) the thickness of compression was positively correlated with AGD / D) and the density of mammary gland was positively correlated with the density of AGD / D (0.263 / P 0.001), and the density of mammary gland was positively correlated with the density of AGD / D / DBT. The thickness of compression and density of breast are independent influencing factors of AGD _ S _ D _ D _ D _ D _ D _ D _ D _ D _ D _ D _ D _ D _ D _ D _ D _ D _ (DBT).
