【摘要】：目的通過動物實驗觀察醫(yī)用膠栓塞實驗兔腎動脈的可行性及安全性,探討不同栓塞劑濃度對血管栓塞過程及效果的影響,為其臨床應用提供指導。方法用不同比例的超液化碘油與醫(yī)用膠組成的混懸液對18只實驗兔腎動脈進行栓塞,觀察栓塞情況及效果。結果栓塞劑經導管注射順利,可經微導管行反復栓塞,安全有效,栓塞可靠,均未見血管再通和造影劑外溢。術后病理檢查HE染色后光學顯微鏡下見動脈內栓塞劑呈不規(guī)則形或分支狀折光性透亮物質,其中,5∶1、4∶1組以細、小動脈栓塞為主,二者比較差異無統(tǒng)計學意義(P0.05),3∶1組以中型動脈栓塞為主,與5∶1組、4∶1組比較差異有統(tǒng)計學意義(P0.05),腎組織可見充血、變性、凝固性壞死、鈣化及纖維組織增生等不同改變。術后腎功能呈一過性損害,白細胞一過性升高,1周后均恢復至術前水平,術后肝功能未見明顯異常。結論醫(yī)用膠栓塞實驗兔腎動脈可行,不同濃度的醫(yī)用膠可以選擇性栓塞腎動脈各級分支。
[Abstract]:Objective to observe the feasibility and safety of medical glue embolization of renal artery in rabbits, and to explore the influence of different concentrations of embolic agent on the process and effect of embolization in order to provide guidance for its clinical application.Methods the renal arteries of 18 rabbits were embolized with different proportion of superliquefied lipiodol and medical glue.Results the embolization agent was successfully injected through the catheter and could be repeatedly embolized through microcatheter. The embolization was safe and effective, and the embolization was reliable. There was no recanalization of blood vessel and no contrast agent overflow.After HE staining, the intraarterial embolization agent showed irregular or branched refractive light transparent substance under optical microscope, in which 5: 1 + 4: 1 group was fine, and arteriole embolization was the main one.There was no significant difference between the two groups (P 0.05) and moderate arterial embolization was dominant in group 3: 1, and there was significant difference compared with group 4: 1 at 5:1 (P 0.05). Congestion, degeneration and coagulation necrosis were observed in renal tissue.Calcification and fibrous tissue proliferation were different.After operation, renal function showed transient damage, leukocyte increased temporarily and recovered to preoperative level 1 week later. Liver function was not abnormal after operation.Conclusion it is feasible to embolize the renal artery of rabbits with medical glue, and different concentrations of medical glue can selectively embolize all branches of renal artery.
