【摘要】：[目的]探討過度運(yùn)動(dòng)致膝關(guān)節(jié)滑膜和軟骨損傷的形態(tài)改變及其損傷機(jī)理。[方法]將25只健康成年Wistar雄性大鼠隨機(jī)分為正常對(duì)照組(對(duì)照組)與過度運(yùn)動(dòng)訓(xùn)練(跑步)組(訓(xùn)練組),分別于1、2、3、6周時(shí)處死大鼠,取全膝關(guān)節(jié),4%PFA(多聚甲醛)液固定、CT掃描、石蠟包埋、切片、染色;通過肉眼觀察、CT成像、Masson三色膠原、Safranin-O、ED1、α-SMA等(免疫)染色觀察滑膜、軟骨形態(tài)學(xué)變化;并用病理圖像、半定量法、Mankin's評(píng)分等進(jìn)行測(cè)定并分析其組織病理學(xué)改變。[結(jié)果]訓(xùn)練組膝關(guān)節(jié)軟骨表面破壞嚴(yán)重,軟骨下骨骨密度明顯增高。Mankin's評(píng)分對(duì)照組(0.43±0.3)分,訓(xùn)練組(5.9±2.1)分(15 km),(7.9±2.7)分(30km);滑膜纖維性病變,對(duì)照組Collgen fibers陽性膠原纖維(9.7±2.3)%,訓(xùn)練組(34.5±3.7)%(15 km),(67.5±4.2)%(30 km);對(duì)照組ED1陽性細(xì)胞半定量(4.8±1.7)分,訓(xùn)練組為(17.3±3.5)個(gè)/mm2(10 km),(13.46±2.7)個(gè)/mm2(30 km);對(duì)照組α-SMA陽性細(xì)胞半定量(92.3±13.6)分,訓(xùn)練組為(287.2±21.3)個(gè)/mm2(5 km),(441.4±100.4)個(gè)/mm2(30 km);對(duì)照組與訓(xùn)練組軟骨及滑膜的病理改變差異具有統(tǒng)計(jì)學(xué)意義(*P0.05;**P0.01)。[結(jié)論]反復(fù)過度運(yùn)動(dòng)能導(dǎo)致膝關(guān)節(jié)滑膜炎、滑膜纖維化以及進(jìn)行性關(guān)節(jié)軟骨破壞等病理改變,并最終發(fā)展為骨關(guān)節(jié)炎。
[Abstract]:[objective] to investigate the morphological changes and injury mechanism of knee joint synovial and cartilage injury induced by overexercise. [methods] Twenty-five healthy adult Wistar rats were randomly divided into normal control group (control group) and overexercise training. The rats in the training group were killed at 6 weeks, The total knee joint was fixed with PFA (paraformaldehyde) fluid for CT scanning, paraffin embedded, sectioned and stained, and Masson trichromatic collagen (Safranin-Ofen ED1, 偽 -SMA et al.) staining for synovium and cartilage morphology were observed by naked eye, and pathological images were used. [results] in the training group, the cartilage surface of the knee joint was seriously damaged, the bone mineral density of the subchondral bone was significantly increased. Mankinos score was 0.43 鹵0.3 in the control group. In the training group, Collgen fibers positive collagen fibers were found in the control group (9.7 鹵2.3), and in the training group (34.5 鹵3.7), 67.5 鹵4.20.The ED1 positive cells in the control group were semi-quantitative (4.8 鹵1.7), and in the training group, 17.3 鹵3.5) rm210kmm-1, 13.46 鹵2.7mm230kmg, respectively, in the control group, and in the control group, the positive cells of 偽 -SMA-positive cells were semi-quantified at 92.3 鹵613.30 kmg, respectively. In the training group, there were 287.2 鹵21.3 / 5kmm-1 / s / mm2 / s / s / mm-1 / 100.4 / s / mm2 / s / mm2, respectively. The pathological changes of cartilage and synovium in the control group and the training group were significantly different. [conclusion] repeated overexercise can lead to synovitis of the knee joint, and the pathological changes of the cartilage and synovium in the control group and the training group are significantly different. Synovial fibrosis and progressive articular cartilage destruction and other pathological changes, and eventually develop into osteoarthritis.
