【摘要】：目的探討布氏桿菌性脊柱炎(BS)的CT及MRI表現(xiàn),以提高對本病的診斷及鑒別診斷水平。方法回顧性分析2013年至2015年本院經(jīng)臨床和/或病理證實(shí)的40例BS患者CT及MRI表現(xiàn)特點(diǎn)。結(jié)果 BS可侵犯脊柱任何部位,以腰椎最多,最易累及L4椎體,受累椎體骨膜增厚,邊緣性骨質(zhì)破壞伴有骨質(zhì)增生硬化,使椎體邊緣呈"花邊椎"改變,小破壞區(qū)無死骨,破壞范圍較大時可見死骨,椎體形態(tài)變化不大,椎間盤破壞輕,椎周膿腫局限無流注,其內(nèi)無鈣化,多不累及腰大肌。結(jié)論 BS的影像學(xué)表現(xiàn)具有特征性,CT及MRI能為BS的診斷提供影像學(xué)依據(jù),具有較高臨床價值。
[Abstract]:Objective to investigate the CT and MRI findings of brucellosis spondylitis. Methods the CT and MRI features of 40 patients with BS confirmed by clinical and / or pathological findings from 2013 to 2015 were analyzed retrospectively. Results BS could invade any part of the spine, and lumbar vertebrae was the most common. The most easily involved L4 vertebrae was the periosteum thickening of the involved vertebral body, the marginal bone destruction accompanied by hyperosteogenesis and sclerosis, which made the edge of the vertebral body appear as "lace vertebrae". There was no dead bone in the small damaged area, and the dead bone could be seen when the damaged area was larger, and the vertebral body morphology was not changed much. Disc destruction was mild, perivertebral abscess was localized without drainage, calcification was not found, and psoas major muscle was not involved. Conclusion CT and MRI features of BS can provide imaging basis for diagnosis of BS and have high clinical value.
