【摘要】：目的:探討256層iCT全景血管成像在評估血液透析患者自體內(nèi)瘺血管功能不全中的價值。方法:36例血液透析并血管通路功能不全患者采用256層iCT進行造瘺側(cè)上肢血管成像,采用對比劑追蹤及智能觸發(fā)掃描技術(shù),對原始圖像采用容積再現(xiàn)(VR)、最大密度投影(MIP)、曲面重組(CPR)等技術(shù)進行后處理,獲取內(nèi)瘺血管的二維及三維圖像。結(jié)果:36例患者均順利完成CT血管成像檢查,能清晰顯示內(nèi)瘺血管全程,內(nèi)瘺血管并發(fā)癥,病變部位、范圍、狹窄程度及側(cè)支循環(huán)形成情況。36例患者中單純流入性動脈狹窄4例,其中多發(fā)節(jié)段性狹窄2例;單純吻合口狹窄7例;單純流出性靜脈狹窄6例;流入性動脈+吻合口狹窄6例,吻合口+流出性靜脈狹窄6例,流入性動脈+吻合口+流出性靜脈狹窄4例,流入性動脈+流出性靜脈狹窄3例。36例患者中合并附壁血栓16例,引流靜脈瘤樣變或節(jié)段性擴長、擴張8例,串珠樣改變4例,9例患肢軟組織腫脹伴發(fā)不同程度側(cè)支循環(huán)形成。結(jié)論:全景CTA可以清晰顯示瘺血管全程,能對內(nèi)瘺血管的并發(fā)癥進行準(zhǔn)確診斷,為臨床治療提供指導(dǎo)。
[Abstract]:Objective: to evaluate the value of 256-layer iCT panoramic angiography in evaluating vascular insufficiency of fistula in hemodialysis patients. By using contrast agent tracing and intelligent trigger scanning technology, the original image was processed by volume reproducing VRV, maximum density projection MIPX, curved surface recombination and CPR, etc. Results CT angiography was successfully completed in 36 patients with internal fistula, which could clearly show the whole course of internal fistula, the complications of internal fistula, the location and range of lesion. The degree of stenosis and the formation of collateral circulation in 36 cases were only inflow stenosis in 4 cases, including multiple segmental stenosis in 2 cases, simple anastomotic stenosis in 7 cases, simple outflow vein stenosis in 6 cases; There were 6 cases of anastomotic stenosis of inflow artery, 6 cases of anastomotic outflow vein stenosis, 4 cases of inflow artery anastomotic outflow vein stenosis, 3 cases of inflow artery outflow vein stenosis, and 16 cases of mural thrombosis. The drainage vein was tumor-like or segmental dilated in 8 cases and beaded in 4 cases with soft tissue swelling and collateral circulation formation in 9 cases. Conclusion: panoramic CTA can clearly show the whole course of fistula. It can accurately diagnose the complications of internal fistula and provide guidance for clinical treatment.
