三代雙源雙能量CT新虛擬單能量技術在旋股內(nèi)、外側(cè)動脈成像中的應用價值

【摘要】:背景與目的評價三代雙源雙能量CT新虛擬單能量技術(advanced virtual monoenergetic technique,mono-plus)提高旋股內(nèi)、外側(cè)動脈成像質(zhì)量的價值。材料與方法收集40例行三代雙源雙能量CT腹部動脈CTA增強檢查(自髂前上棘水平至股骨中段掃描)患者圖像,采用雙能量模式(90/150SnkVp)掃描,重建后的影像數(shù)據(jù)均預先用設備自帶Syngo.via VA30A,Siemens工作站進行后處理,通過Monoenernetic Plus功能獲得40keV mono-plus圖像(組1),Optimum Contrast功能獲得非線性融合圖像(組2),最佳CNR單能量曲線(optimal CNR)獲得最佳kev單能量圖像(組3)。掃描時獲得混合能量(Mixed,混合系數(shù)0.5)圖像(組4)。分別計算并比較四組圖像中股動脈(一級動脈)、股深動脈(二級動脈)、旋股外側(cè)動脈(三級動脈)、旋股外側(cè)動脈橫支(四級動脈)、旋股內(nèi)側(cè)動脈(三級動脈)、旋股內(nèi)側(cè)動脈深支(四級動脈)的CT值、信噪比(SNR)、對比噪聲比(CNR)以及圖像質(zhì)量主觀評分。結果對40例患者定量分析結果顯示,總四級動脈分支的CT值都呈現(xiàn)出40keV mono-plus組最高,混能組最低(P0.05)。雖然40keV mono-plus組的總四級動脈分支的SNR值均沒有非線性融合圖像高(P0.05),但第一、二級動脈分支的SNR值仍高于混能組(P0.05),且與最佳單能組沒有統(tǒng)計學差異(P0.05),第三、四級動脈分支的SNR值與混能組、最佳單能組均無統(tǒng)計學差異(P0.05)。在40keV mono-plus組的SNR值良好的基礎上,其CNR值于總四級動脈分支均顯著優(yōu)于其余三組圖像(P0.01),并且其主觀評分也明顯優(yōu)于其余三組圖像。結論在髂腹股溝區(qū)動脈不同重建模式圖像的顯示上,40keV mono-plus模式重建圖像質(zhì)量明顯優(yōu)于非線性融合、最佳單能量以及混合能量模式重建圖像,除股動脈、股深動脈比其余三組圖像顯示更好之外,旋股內(nèi)、外側(cè)動脈的遠端及其細小的動脈分支也能清晰顯示,推薦其作為股骨頭供血動脈DE-CTA掃描的常規(guī)重建模式。
[Abstract]:Background & AIM to evaluate the new virtual single energy technique of three generations dual source and dual energy CT (DECT) with advanced virtual monoenergetic technique mono-plus). Materials and methods the images of 40 patients (from the anterior superior iliac spine level to the middle femur) were collected in 40 patients with double source and dual energy CT abdominal artery CTA enhancement (from the level of anterior superior iliac spine to the middle femur), and were scanned by double energy mode 90 / 150SnkVp. The reconstructed image data were pre-processed with Syngo.via VA30A Siemens workstation. 40 Kev mono-plus images obtained by Monoenernetic Plus function (group 1 optimum Contrast function to obtain nonlinear fusion image (group 2, optimal CNR single energy curve) to obtain optimal kev single energy image (group 3. Mixed energy, mixing coefficient 0. 5). (group 4) calculate and compare the femoral artery (primary artery, deep femoral artery, lateral femoral circumflex artery, lateral femoral circumflex artery, lateral femoral circumflex artery, lateral femoral circumflex artery, medial femoral circumflex artery, medial femoral circumflex artery) in the four groups. Ct value of the deep branch of medial circumflex femoral artery (fourth grade artery), Results the results of quantitative analysis in 40 patients showed that the CT values of the branches of the total four-grade arteries were the highest in the 40keV mono-plus group. Although the SNR value of the total fourth grade artery in the 40keV mono-plus group was not as high as that of the nonlinear fusion image, the SNR value of the secondary artery branch was still higher than that of the mixed energy group (P0.05), and there was no statistical difference between the two groups and the optimal monoenergetic group (P0.05), third, there was no statistical difference between the two groups. There was no statistical difference between the SNR value of the four-grade artery branch and the mixed energy group, and there was no statistical difference in the best monoenergetic group. On the basis of the good SNR value of the 40keV mono-plus group, Its CNR value was significantly better than that of the other three groups (P 0.01) and its subjective score was significantly better than that of the other three groups. Conclusion the images of different reconstruction modes of ilioinguinal artery are displayed in 40keV mono-plus mode. The image quality is obviously superior to that of nonlinear fusion. In addition to the femoral artery, the deep femoral artery and the deep femoral artery showed better than the other three groups. The distal end of the circumflex femoral artery and the lateral artery and its small branches could also be clearly displayed. It is recommended as a conventional reconstruction model for DE-CTA scan of femoral head supplying artery.
