基于IMRT和VMAT的自適應(yīng)優(yōu)化算法的研究與應(yīng)用

【摘要】:調(diào)強(qiáng)放射治療(Intensity Modality Radiation Therapy, IMRT)和容積弧形調(diào)強(qiáng)治療(Volumetric ModalityArc Therapy,VMAT)是目前放射治療中所采用的主流技術(shù)。論文通過(guò)對(duì)IMRT、VMAT、及采用該兩項(xiàng)技術(shù)的放射治療方案在臨床中應(yīng)用現(xiàn)狀的分析,針對(duì)IMRT和VMAT在放射治療中所存在的不足,提出了一種基于IMRT和VMAT的自適應(yīng)優(yōu)化算法(Adaptive Optimization Program,AOP)。通過(guò)對(duì)采用該算法的放射治療方案在仿真人體模上的實(shí)驗(yàn)和實(shí)驗(yàn)結(jié)果的分析,驗(yàn)證其在臨床上的可行性和可推廣性。論文的研究工作主要有以下幾個(gè)方面: 1.通過(guò)對(duì)IMRT和VMAT技術(shù)的分析,指出其在放射治療中所存在的不足。IMRT治療復(fù)雜、每個(gè)照射野上附帶子野過(guò)多、放療機(jī)執(zhí)行時(shí)間長(zhǎng)、效率低、缺乏人性化的考慮;VMAT治療速度快效率高,但在放療方案的優(yōu)化中具妥協(xié)性、過(guò)多的全身照射范圍使病人體部低劑量照射區(qū)域大。 2.針對(duì)IMRT和VMAT在放射治療中所存在的不足,,通過(guò)對(duì)放射治療方案在臨床中應(yīng)用現(xiàn)狀的分析,提出一種基于IMRT和VMAT的優(yōu)化算法。采用該算法的放射治療方案既保留了高精度,又具有較高的效率,且總子野數(shù)少于IMRT/VMAT。 3.對(duì)采用AOP的放射治療方案在仿真人體模上進(jìn)行仿真實(shí)驗(yàn):在仿真人體模頭頸部、腹部腫瘤的虛擬病例下測(cè)試成功,符合臨床要求。 4.通過(guò)對(duì)仿真實(shí)驗(yàn)及其實(shí)驗(yàn)結(jié)果的分析,驗(yàn)證該算法在臨床上的可行性與實(shí)用性。本文提出的AOP及采用該算法的放射治療方案在仿真環(huán)境下得到了較好的臨床實(shí)驗(yàn)效果。實(shí)驗(yàn)結(jié)果表明AOP在仿真人體模上有臨床可實(shí)現(xiàn)性且具備向臨床推廣的可能性。
[Abstract]:Intensity Modality Radiation Therapy (IMRT) and volume intensity modulated intensity modulated ModalityArc (VRT) are the main techniques used in radiotherapy at present. In view of the shortcomings of IMRT and VMAT in radiotherapy, an adaptive Optimization program AOPO algorithm based on IMRT and VMAT is proposed. The experimental results and experimental results of radiation therapy schemes based on this algorithm in simulated human models are analyzed. To verify its clinical feasibility and extensibility. The research work of this paper mainly includes the following aspects:. 1. Through the analysis of IMRT and VMAT technology, it is pointed out that the shortcomings of IMRT and VMAT in radiotherapy are complex. Each irradiation field has too many sub-fields, the time of radiotherapy machine is long, and the efficiency of radiotherapy machine is low. In the absence of humanized consideration, VMAT treatment is fast and efficient, but it is compromise in the optimization of radiotherapy scheme, too much whole body irradiation range makes the low dose irradiation area of patient's body larger. 2. In view of the deficiency of IMRT and VMAT in radiotherapy, an optimization algorithm based on IMRT and VMAT is proposed by analyzing the present situation of clinical application of radiotherapy. It also has high efficiency, and the total number of subfields is less than IMRT / VMATT. 3. The emulation experiment was carried out on the simulated human model with the radiation therapy scheme of AOP: the virtual cases of head and neck and abdominal tumors were successfully tested, which met the clinical requirements. 4. The simulation experiment and its results are analyzed. The feasibility and practicability of the algorithm in clinic are verified. The proposed AOP and the radiation therapy scheme using the algorithm have achieved good clinical experimental results in the simulation environment. The experimental results show that AOP is applied in the simulation of human model. It has clinical realizability and the possibility of popularizing to clinic.
