【摘要】：目的探討總結(jié)成人Henoch紫癜的螺旋CT影像特點。方法回顧性分析20例Henoch紫癜患者螺旋CT表現(xiàn),對腸壁病變范圍、腸壁形態(tài)、腸管外脂肪間隙及腸系膜改變、腸腔內(nèi)外伴發(fā)征象、強化表現(xiàn)及腹腔積液情況等分析總結(jié)。結(jié)果 20例Henoch紫癜患者中,單一部位受累11例,多部位受累9例。胃受累3例,十二指腸受累6例,空、回腸受累13例,結(jié)腸受累8例。18例病變腸管管壁彌漫增厚,其中10例腸壁環(huán)形增厚,解剖層次清晰;8例增厚腸壁解剖層次模糊。15例漿膜外腸系膜側(cè)脂肪間隙密度增高。3例腹腔少量積液。10例行CT增強掃描患者中,8例顯示腸壁全層彌漫強化;2例腸壁分層強化。3例結(jié)腸腸腔內(nèi)積血。腸管3~7天腫脹減輕8例;腸管7~14天基本恢復正常17例;3例1個月后恢復。結(jié)果 Henoch紫癜的基本CT征象包括彌漫或多節(jié)段腸管均勻全層或環(huán)形分層增厚,彌漫全層或環(huán)形強化及漿膜外滲出。CT檢查是Henoch紫癜診斷和復查的有效手段。
[Abstract]:Objective to investigate the spiral CT features of adult Henoch purpura. Methods the spiral CT findings of 20 patients with Henoch purpura were retrospectively analyzed. Results among the 20 patients with Henoch purpura, 11 had single site involvement, 9 multiple site involvement, 3 gastric involvement, 6 duodenal involvement, 13 emptying and ileal involvement. 8 cases of colon involvement. 18 cases of pathological changes of intestinal wall diffuse thickening, 10 cases of intestinal wall ring thickening, The anatomical level was clear in 8 cases with thickening intestinal wall. In 15 cases, the density of fat space in extraserous mesenteric side was increased in 3 cases. 3 cases with small amount of peritoneal effusion. 10 cases with CT enhanced scanning. 8 cases showed diffuse enhancement of the whole layer of intestinal wall in 2 cases. In 3 cases, hemorrhage in colon lumen was enhanced by stratification, and swelling of intestinal duct was alleviated in 8 cases in 3 days. Results the basic CT signs of Henoch purpura included diffuse or multisegmental homogeneous full-layer or annular thickening of intestinal canal. Diffuse or ring enhancement and extraserous exudation. Ct is an effective method for diagnosis and reexamination of Henoch purpura.
