【摘要】：目的將自適應(yīng)迭代重建技術(shù)(ASi R)用于肥胖病人的腎上腺CT成像,旨在評價ASi R在降低輻射劑量和提高影像質(zhì)量中的應(yīng)用價值。方法收集200例肥胖病人并行腎上腺CT掃描,男112例,女88例,年齡17~78歲,平均(54.22±18.63)歲,分為2組,各100例。A組行常規(guī)劑量的濾過反投影(FBP)重建,B組為降低劑量的ASi R 4個等級(未降低,分別降低30%、50%、100%)重建,所有重建圖像傳送至AW4.6工作站,分別由3名CT診斷醫(yī)師采用雙盲法對兩組影像質(zhì)量進(jìn)行評價。采用t檢驗(yàn)對比兩組輻射劑量,單因素方差分析比較影像質(zhì)量的客觀指標(biāo)(噪聲、SNR、CNR)及主觀評分,組間多重比較采用LSD-t檢驗(yàn)。采用Kappa檢驗(yàn)評價3名醫(yī)師主觀評分一致性。結(jié)果 A組與B組4個不同迭代等級的重建影像進(jìn)行比較,B組中各重建的腎上腺噪聲、肌肉噪聲及脂肪噪聲均隨迭代等級升高而下降,SNR及CNR隨迭代等級的升高而升高(P0.05)。其中,ASi R50%時各部位噪聲、腎上腺SNR與FBP重建相近(P0.05)。3名醫(yī)師主觀評分為一致性較好(Kappa=0.76)。影像質(zhì)量顯示能力評分由高到低依次為ASi R100%、A組FBP、ASi R50%、ASi R30%、ASi RNone(P0.05)。結(jié)論在肥胖病人腎上腺CT成像中應(yīng)用ASi R技術(shù)顯著降低了輻射劑量,ASi R(50%~100%)所得影像質(zhì)量近似或優(yōu)于傳統(tǒng)FBP。
[Abstract]:Objective to evaluate the value of ASi R in reducing radiation dose and improving image quality in adrenal gland CT imaging of obese patients using adaptive iterative reconstruction technique (ASI R). Methods 200 obese patients were examined with adrenal CT scan. 112 males and 88 females, aged 1778 years, with an average age of 54.22 鹵18.63 years, were divided into two groups. Each group (100 cases) was reconstructed with conventional dose of FBP (FBP). Group B had four grades of ASi R with low dose (no decrease, with a reduction of 30 ~ 50% and 100%). All the reconstructed images were transmitted to the AW4.6 workstation, and the quality of the two groups was evaluated by three CT diagnostics using double-blind method. T test was used to compare the radiation doses between the two groups. Single factor ANOVA was used to compare the objective index (noise SNR CNRR) and subjective score of image quality. LSD-t test was used for multiple comparisons among groups, and Kappa test was used to evaluate the consistency of subjective scores of 3 physicians. Results the reconstructed images of group A and group B were compared with each other in 4 different iterations. The muscle noise and fat noise decreased with the increase of iterative grade and CNR increased with the increase of iterative grade. The subjective scores of adrenal SNR and FBP reconstruction were similar to that of P0.053.The subjective scores of the doctors were good. The imaging quality scores were ASi R100 and A in turn from high to low. Conclusion ASi R technique is used in the adrenal CT imaging of obese patients. The image quality is similar to or superior to that of traditional FBPs.
