數字胃腸機在食管癌門診機會性篩查中的應用

【摘要】:目的探討數字胃腸機在食管癌門診機會性篩查中的應用價值。方法在甘肅省武威腫瘤醫(yī)院2011-01-01-2013-10-31門診就診的人群中進行食管癌篩查,所有接受篩查者在檢查前簽署知情同意書,按篩查流程進行癌癥防治知識和食管癌危險因素流行病學調查,詳細詢問病史及一般體格檢查。采用大平板數字胃腸機(德國西門子AXIOM luminos dRF)氣鋇雙重造影,發(fā)現(xiàn)食管有病變者均相對應地行內鏡檢查,最終確診均以病理組織學為依據。結果共用數字胃腸機檢查2 599人,其中男1 747人,女852人;40~69歲2 058人,占受檢人數的79.18%。檢出食管癌165例,檢出率為6.35%。早期食管癌4例(2.42%),中晚期食管癌161例(97.58%)。其中男129例,檢出率為7.38%;女36例,檢出率為4.23%。男女性檢出率差異有統(tǒng)計學意義,χ2=9.61,P0.01。食管癌數字胃腸造影檢查與病理診斷比較,靈敏度為98.19%,特異度為99.68%。年齡組檢出率有隨年齡的增長而升高的趨勢,55~80歲組占總檢出人數的93.94%。結論數字胃腸造影機會性篩查較之自然人群篩查,食管癌檢出率高,是食管癌篩查的有效手段,作為食管癌的初篩方法具有可行性!55歲的人群應為食管癌防治的重點。
[Abstract]:Objective to explore the application value of digital gastrointestinal machine in the screening of esophageal cancer outpatient. Methods it was carried out in the outpatient population of Wuwei Cancer Hospital, Gansu Province, 2011-01-01-2013-10-31. Esophageal cancer screening. According to the screening procedure, all the screening participants signed an informed consent form and carried out an epidemiological survey of cancer prevention and control knowledge and the risk factors of esophageal cancer. The history and general physical examination were inquired in detail. Barium double radiography was performed with AXIOM luminos dRFs. It was found that the patients with esophageal lesions were examined by endoscopy, and the final diagnosis was based on histopathology. Results 2 599 patients were examined by digital gastrointestinal machine, including 1 747 males and 852 females. 4069 years old 2058 people, accounting for 79.18. 165 cases of esophageal cancer were detected, the detection rate was 6.35%. Early esophageal carcinoma 4 cases (2.42%). 161 cases of advanced esophageal carcinoma, including 129 cases of male, the detection rate was 7.38%; 36 cases were female, the detection rate was 4.23.The difference between male and female was statistically significant (蠂 ~ 2 / 9.61 / P0.01.Compared with pathological diagnosis of esophageal carcinoma, digital gastrointestinal angiography). The sensitivity was 98.19 and the specificity was 99.68.The detection rate in the age group increased with the increase of age. Conclusion compared with the natural population, the detection rate of esophageal cancer is higher in the group of 5580 years old than that in the natural population, which is an effective means of esophageal cancer screening.
