【摘要】：目的總結(jié)胰腺外傷和相關(guān)并發(fā)癥及其在隨訪復(fù)查中的多層螺旋CT(MSCT)影像表現(xiàn)。方法回顧性分析28例經(jīng)臨床確診的胰腺外傷患者M(jìn)SCT檢查資料,參考美國創(chuàng)傷外科學(xué)分會(huì)(AAST)提出的分級(jí)標(biāo)準(zhǔn),對(duì)胰腺損傷進(jìn)行分級(jí)診斷,觀察胰腺不同損傷等級(jí)和相關(guān)并發(fā)癥及其在隨訪復(fù)查中的MSCT影像表現(xiàn)。結(jié)果首診時(shí)漏診3例,25例首診時(shí)可見胰腺損傷的直接征象而明確診斷,28例可見胰腺損傷間接征象。Ⅰ級(jí)損傷8例,Ⅱ級(jí)損傷6例,Ⅲ級(jí)損傷7例,Ⅳ級(jí)損傷5例,Ⅴ級(jí)損傷2例。增強(qiáng)掃描能清晰顯示胰腺損傷范圍及斷裂帶,斷裂帶超過50%者術(shù)中可見主胰管斷裂傷。胰腺損傷相關(guān)并發(fā)癥包括假性囊腫9例,胰周血管損傷6例,腹腔或胰周感染2例,胰腺萎縮3例。結(jié)論 MSCT能及時(shí)發(fā)現(xiàn)胰腺損傷,增強(qiáng)掃描可明顯提高胰損傷診斷的陽性率,更準(zhǔn)確地評(píng)估胰腺損傷程度及分級(jí)。MSCT隨訪可觀察胰腺恢復(fù)情況,有無假性囊腫形成及囊腫的大小變化、腹腔或胰周感染、胰腺萎縮等并發(fā)癥,為臨床醫(yī)生選擇合理的治療方案提供依據(jù)。
[Abstract]:Objective to summarize the pancreatic trauma and related complications and the MSCT of multislice spiral CTT in the follow-up and reexamination of pancreatic trauma. Methods the MSCT findings of 28 patients with pancreatic trauma were retrospectively analyzed. According to the classification standard proposed by AAST, the classification diagnosis of pancreatic injury was carried out. To observe the different grades of pancreatic injury and related complications and their MSCT imaging findings in follow-up and reexamination.
