【摘要】：目的探討X線(xiàn)分型在老年人Colles骨折治療策略選擇及預(yù)后評(píng)估中的價(jià)值。方法對(duì)44例60歲以上Colles骨折患者進(jìn)行縱向回顧性研究。根據(jù)Frykman分型將病例分為關(guān)節(jié)內(nèi)型骨折組(26例)和關(guān)節(jié)外型骨折組(18例)。關(guān)節(jié)內(nèi)型骨折組接受保守治療11例,手術(shù)治療15例;關(guān)節(jié)外型骨折組接受保守治療10例,手術(shù)治療8例。所有患者在受傷后治療前、治療后1周、2周、6~8周均行腕關(guān)節(jié)X線(xiàn)平片檢查。測(cè)量所有患者復(fù)位前后X線(xiàn)平片的橈骨掌傾角、橈骨尺偏角、橈骨短縮值。骨折愈合的影像學(xué)結(jié)果應(yīng)用Lidstrom分級(jí)進(jìn)行評(píng)價(jià)。統(tǒng)計(jì)并比較兩組患者保守治療、手術(shù)治療結(jié)果。結(jié)果按照Lidstrom分級(jí),44例Colles骨折患者經(jīng)治療后75.0%的影像學(xué)結(jié)果可接受。其中關(guān)節(jié)內(nèi)型骨折組與關(guān)節(jié)外型骨折組可接受率分別為76.9%、72.2%,兩組比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P=0.723)。關(guān)節(jié)內(nèi)型骨折組保守治療與手術(shù)治療的可接受率分別為54.5%和93.3%,兩種治療方式差異有統(tǒng)計(jì)學(xué)意義(P0.05);關(guān)節(jié)外型骨折組保守治療與手術(shù)治療的可接受率分別為60.0%和87.5%,兩種治療方式差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論老年人Colles骨折患者,關(guān)節(jié)內(nèi)型手術(shù)治療效果優(yōu)于保守治療,關(guān)節(jié)外型保守治療與手術(shù)治療效果相仿。
[Abstract]:Objective to investigate the X-ray type fracture treatment evaluation strategies and prognosis in elderly Colles patients. Methods 44 patients above 60 years old Colles fracture of longitudinal review patients. According to the Frykman classification, the cases were divided into intra-articular fracture group (26 cases) and the joint appearance of fracture group (18 cases) joint. In the fracture group received conservative treatment in 11 cases, 15 cases of surgical treatment; joint appearance fracture group received conservative treatment in 10 cases, 8 cases of surgical treatment. All patients after injury before 1 weeks, after 2 weeks of treatment, 6~8 Zhou Junhang wrist X-ray measurement. All the patients before and after treatment of X-ray the radial palmar tilt, radial ulnar deviation, radial shortening value. To evaluate the fracture healing of the imaging results using Lidstrom classification. Statistics and compared two groups of patients with conservative treatment, surgical treatment results. Results according to Lidstrom classification, 44 cases of Colles fracture patients after treatment 7 5% the imaging results are acceptable. The intra articular fracture group and fracture group joint appearance acceptable rates were 76.9%, 72.2%, two groups had no statistically significant difference (P=0.723). Intra articular fracture group of conservative treatment and surgical treatment acceptable rates were 54.5% and 93.3%, there were significant differences between the two the treatments (P0.05); joint fracture group with conservative treatment and surgical treatment acceptable rates were 60% and 87.5%, there was no difference among the two treatments (P0.05). Conclusion the elderly patients with Colles fracture, intra-articular surgery is better than conservative treatment, the treatment effect of conservative treatment and surgical joint appearance similar.
