【摘要】：目的了解南京市腎綜合征出血熱(HFRS)流行病學(xué)特征,為制定有效的防控策略和措施提供科學(xué)依據(jù)。方法采用描述流行病學(xué)方法,分析2011-2016年南京市報(bào)告的HFRS病例流行特征,以及病例臨床特征和可疑暴露信息等。結(jié)果 2011-2016年南京市共報(bào)告HFRS確診病例194例,年均32例(19~45例),年均報(bào)告發(fā)病率為0.39/10萬(0.23/10萬~0.55/10萬)。季節(jié)上呈現(xiàn)雙峰分布,春季(3~6月)峰值高于冬季峰(12月~次年1月)。男性多于女性,病例年齡中位數(shù)為48歲(12~82歲),農(nóng)民占57%。高淳區(qū)報(bào)告病例數(shù)占全市的45%(87/194),年均報(bào)告發(fā)病率為3.4/10萬(1.9/10萬~4.8/10萬)。發(fā)病存在空間正相關(guān)(Moran's I=0.17,P0.05),高淳區(qū)(2012年4月1日-2015年3月31日)和江寧區(qū)(2013年1月1日-2013年5月31日)存在時(shí)空聚集。病例多具有發(fā)熱、"三痛","三紅"等較典型癥狀或體征,但有明確暴露史者比例極低。結(jié)論南京市HFRS發(fā)病平穩(wěn),應(yīng)根據(jù)流行特征在重點(diǎn)地區(qū)和人群開展綜合防控措施。
[Abstract]:Objective to investigate the epidemiological characteristics of hemorrhagic fever with renal syndrome (HFRS) in Nanjing and provide scientific basis for effective prevention and control strategy. Methods descriptive epidemiology was used to analyze the epidemiological characteristics of HFRS cases reported in Nanjing from 2011 to 2016, as well as the clinical characteristics and suspicious exposure information. Results A total of 194 HFRS cases were reported in Nanjing from 2011 to 2016, with an annual average of 32 cases (1945 cases) and an annual reported incidence rate of 0.39 / 100 000 (0.23 / 100 000 ~ 0.55 / 100 000). The peak value in spring (from March to June) is higher than that in winter (December to January of next year). The median age of the cases was 48 years (1282 years), with farmers accounting for 57%. The reported cases in Gaochun accounted for 45% (87 / 194) of the city, with an average annual reported incidence of 3.4 / 100 thousand (19.9 / 100,000-4.8 / 100,000). There was a positive spatial correlation (Moran's), a spatiotemporal aggregation in Gaochun (from April 1, 2012 to March 31, 2015) and Jiangning District (from January 1, 2013 to May 31, 2013). Most of the cases had fever, three pain, three red and other typical symptoms or signs, but the proportion of patients with definite exposure history was very low. Conclusion the incidence of HFRS in Nanjing is stable, and comprehensive prevention and control measures should be carried out in key areas and populations according to epidemic characteristics.
