【摘要】：據統計,到2050年,全球老年人口將增至目前的3倍,達20億。由于長期以來我國出生人口比例一直偏低,老齡人口比例逐步增高,加之人們對健康的普遍重視,急性醫(yī)療系統將面臨缺乏足夠的醫(yī)療設施和專業(yè)的醫(yī)療隊伍、缺乏對多病共存疾病的有效治療等難題。同時急性醫(yī)療系統對需長期住院的老年患者的醫(yī)源性傷害問題,如肌少癥的發(fā)生、衰弱與失能的加速等也日益受到重視。因此為了緩解急性醫(yī)療系統的壓力,滿足老年患者多樣化的需求,使老年患者能夠得到更加科學合理的照料,一種新的照護模式,即中期照護模式逐漸發(fā)展起來。中期照護通過促進患者的功能自主達到最佳狀態(tài)以實現其主要目標:促進患者及時出院、避免不必要的再住院及減少入住長期照護機構的需求。本文就中期照護的定義、特點、流程及我國發(fā)展中期照護的建議進行綜述,為進一步研究中期照護及相關衛(wèi)生政策的制定提供一定的依據。
[Abstract]:According to statistics, by 2050, the world's elderly population will triple to 2 billion. Because the proportion of birth population has been low for a long time in our country, the proportion of elderly population is increasing gradually, and people pay more attention to health, the acute medical system will be faced with the lack of adequate medical facilities and professional medical team. Lack of effective treatment for multiple diseases and other problems. At the same time, the problem of iatrogenic injury in the acute medical system to the elderly patients who need to be hospitalized for a long time, such as the occurrence of myasthenia, the acceleration of weakness and disability, has been paid more and more attention. Therefore, in order to relieve the pressure of acute medical system, to meet the diverse needs of elderly patients and to enable the elderly patients to get more scientific and reasonable care, a new mode of care, that is, the medium-term care model, has been gradually developed. Medium-term care achieves its main goal by promoting the patient's functional autonomy to achieve its main goal: to promote the timely discharge of patients, to avoid unnecessary re-hospitalization and to reduce the need for admission to long-term care institutions. In this paper, the definition, characteristics, flow chart and suggestions for the development of intermediate care in China are reviewed, which provides a basis for further study on medium-term care and the formulation of related health policies.
