【摘要】：目的探討人工流產(chǎn)術(shù)后使用短效口服避孕藥（優(yōu)思明）、宮內(nèi)放置節(jié)育器及男式避孕套避孕的臨床效果。 方法來(lái)我院進(jìn)行人工流產(chǎn)術(shù)并自愿術(shù)后立即口服避孕藥的65例女性列為優(yōu)思明組，，術(shù)后選擇放置宮內(nèi)節(jié)育器的69例作為IUD組，術(shù)后選擇男式陰莖套避孕的58例為避孕套組。隨訪6個(gè)月，觀察三組術(shù)后陰道出血時(shí)間、轉(zhuǎn)經(jīng)時(shí)間、術(shù)后月經(jīng)量的變化及不良反應(yīng)等情況進(jìn)行臨床對(duì)比。 結(jié)果 三組患者人流術(shù)后出血時(shí)間相比較，優(yōu)思明組和陰莖套組陰道出血時(shí)間明顯短于IUD組（P0.05）；三組患者術(shù)后月經(jīng)恢復(fù)時(shí)間的比較均有顯著差異（P0.05），和其他兩組相比，優(yōu)思明組術(shù)后首次月經(jīng)轉(zhuǎn)歸時(shí)間較短。術(shù)后三組患者月經(jīng)量的變化情況比較有顯著差異（P＜0.05），優(yōu)思明組術(shù)后月經(jīng)量小于以往月經(jīng)量的患者有63.0%，多于陰莖套組和IUD組，IUD組術(shù)后月經(jīng)量大于以往月經(jīng)量的患者有53.6%，多于優(yōu)思明組和陰莖套組。優(yōu)思明組與IUD組不良反應(yīng)率的比較差異有統(tǒng)計(jì)學(xué)意義（P＜0.05），IUD組的不良反應(yīng)發(fā)生率為36.2%，優(yōu)思明組不良反應(yīng)發(fā)生率為20.0％。 結(jié)論 1.人工流產(chǎn)術(shù)后即時(shí)服用優(yōu)思明能較快恢復(fù)正常月經(jīng)周期，值得臨床上推廣； 2.IUD組不良反應(yīng)主要有月經(jīng)期延長(zhǎng)、腰困腰酸，但半年后基本恢復(fù)正常，所以可以作為長(zhǎng)期避孕的首選方式。 3.對(duì)于不能口服避孕藥和放置宮內(nèi)節(jié)育器避孕的患者，避孕套可作為首選避孕方式。
[Abstract]:Objective to investigate the clinical effect of using short effect oral contraceptive (Yosemin), intrauterine contraceptive device (IUD) and male condom after artificial abortion. Methods Sixty-five women who underwent artificial abortion in our hospital and volunteered to take oral contraceptive immediately after operation were classified as Yosemin group, 69 cases of intrauterine contraceptive device were selected as IUD group, 58 cases of male penile condom were selected as condom group after operation. After 6 months of follow-up, the time of vaginal bleeding, the time of conversion, the changes of menstrual volume and adverse reactions were observed and compared among the three groups. Result The vaginal bleeding time of the three groups was significantly shorter than that of the IUD group (P 0.05), and the time of menstrual recovery in the three groups was significantly different from that in the other two groups (P 0.05), and compared with the other two groups, the vaginal bleeding time of the three groups was significantly shorter than that of the other two groups, and the time of menstrual recovery was significantly lower in the three groups than in the other two groups. The first menstrual recovery time was shorter in Yousiming group. There was significant difference in menstrual volume among the three groups after operation (P < 0.05). There were 63.0 patients in Yousiming group who had less menstrual volume after operation than those in penile sleeve group and IUD group. 53.6% were more than Yousiming group and penile sleeve group. There was significant difference in the adverse reaction rate between Yousiming group and IUD group (P < 0.05). The incidence of adverse reaction was 36.2 in Yousiming group and 20.0 in Yousiming group. Conclusion 1. Immediately after abortion, you Siming can recover the normal menstrual cycle quickly, which is worth popularizing in clinic. In 2.IUD group, the main adverse reactions were prolonged menstrual period and lumbar acid, but returned to normal half a year later, so it can be used as the first choice for long-term contraception. 3. Condoms may be the first choice for patients who cannot take oral contraceptives and place IUDs.
