【摘要】：目的觀察雙肺同期大容量肺泡灌洗治療塵肺的療效及安全性。方法回顧性分析2013年12月至2015年6月于該院行全身麻醉下雙肺同期大容量肺泡灌洗的153例患者病例資料,比較手術(shù)前后肺功能、血氣分析指標(biāo)、臨床評分,觀察并發(fā)癥情況。結(jié)果與術(shù)前比較,153例患者術(shù)后3個月臨床評分降低,差異有統(tǒng)計學(xué)意義(P0.05);血氣分析指標(biāo)中動脈血氧分壓(PaO_2)升高,動脈血二氧化碳分壓(PaCO_2)降低,差異有統(tǒng)計學(xué)意義(P0.05),而pH值比較,差異無統(tǒng)計學(xué)意義(P0.05);肺功能指標(biāo)中用力肺活量(FVC)、最大通氣量(MVV)、第1秒鐘用力呼氣容積(FEV1)均升高,差異均有統(tǒng)計學(xué)意義(P0.05)。無1例嚴重并發(fā)癥發(fā)生。結(jié)論塵肺患者采用雙肺同期大容量肺泡灌洗治療是一種相對無創(chuàng)的病原療法,其并發(fā)癥少,安全可靠,可改善患者的臨床癥狀,從而提高其生活質(zhì)量。
[Abstract]:Objective to observe the efficacy and safety of large volume alveolar lavage in the treatment of pneumoconiosis. Methods from December 2013 to June 2015, 153 patients with double lung and large volume alveolar lavage under general anesthesia were analyzed retrospectively. Pulmonary function, blood gas analysis index, clinical score and complications were compared before and after operation. Results the clinical scores of 153 patients were significantly lower than those before operation (P 0.05), the arterial partial pressure of oxygen (Pao _ 2) in blood gas analysis was increased, and Paco _ 2 (P _ (CO _ 2) _ 2) was decreased, the difference was statistically significant (P < 0.05), and pH value was higher. There was no significant difference (P 0.05), but forced vital capacity (FV), maximal ventilation volume (MVV) and forced expiratory volume (FEV 1) in the lung function index were all increased, and the difference was statistically significant (P 0.05). No serious complications occurred. Conclusion the treatment of pneumoconiosis with large volume alveolar lavage is a relatively noninvasive pathogen therapy with less complications, safety and reliability. It can improve the clinical symptoms and improve the quality of life of pneumoconiosis patients.
