中小學(xué)生青春發(fā)動(dòng)時(shí)相及影響因素的追蹤研究

【摘要】:目的追蹤觀察鄭州市中小學(xué)生青春發(fā)動(dòng)時(shí)相的現(xiàn)狀及發(fā)動(dòng)提前可能的影響因素,并探討這些因素對(duì)青春發(fā)動(dòng)時(shí)相影響可能的性別差異,為揭示青春發(fā)動(dòng)時(shí)相影響機(jī)制提供依據(jù)。方法本研究選取了鄭州市區(qū)3所學(xué)校的小學(xué)二三年級(jí)、初一二三年級(jí)女生;小學(xué)三四年級(jí)、初一二三年級(jí)男生,于2014年10月進(jìn)行有關(guān)青春期發(fā)育的基線調(diào)查,并分別于2015年10月、2016年10月進(jìn)行后續(xù)的追蹤調(diào)查。此外,本研究還測(cè)量了部分樣本晨尿中的環(huán)境內(nèi)分泌干擾物的代謝產(chǎn)物。結(jié)果1.青春發(fā)動(dòng)時(shí)相追蹤基期年齡為8歲男生三次調(diào)查陰毛發(fā)育率依次為0、0、1%,辜丸發(fā)育達(dá)到Ⅱ期的比例依次為5.9%、12.7%、56.9%;基期年齡為9歲男生三次調(diào)查陰毛發(fā)育率依次為0%、2.0%、11.0%,睪丸發(fā)育達(dá)到Ⅱ期的比例依次為8%、17.3%、75.3%;基期年齡為12歲男生三次調(diào)查陰毛發(fā)育率依次為45.1%、96.1%、100%,睪丸發(fā)育達(dá)到Ⅱ期的比例依次為88.2%、100%、100%;谀挲g為7歲女生三次調(diào)查月經(jīng)來(lái)潮率依次為0%、1.5%、2.3%,乳房發(fā)育達(dá)到Ⅱ期比例依次為18.2%、48.9%、100%;基期年齡為8歲女生三次調(diào)查月經(jīng)來(lái)潮率為0%、0.7%、9.9%,乳房發(fā)育達(dá)到Ⅱ期比例依次為48.9%、57.4%、80.9%;基期年齡為12歲女生三次調(diào)查月經(jīng)來(lái)潮率依次為70.0%、92.0%、97.0%,乳房發(fā)育達(dá)到Ⅱ期比例均為100%。2.青春發(fā)動(dòng)時(shí)相分布追蹤三次調(diào)查男生青春發(fā)動(dòng)時(shí)相提前率依次為17.8%、17.8%、20.3%;三次調(diào)查女生青春發(fā)動(dòng)時(shí)相提前率依次為12.1%、16.1%、21.7%。3.青春期發(fā)育與體格、體脂指標(biāo)分析男生青春期發(fā)育啟動(dòng)年齡與身高、體重、肩胛部皮褶厚度、腹部皮褶厚度之間相關(guān)(r=0.609、r=0.343、r=0.209、r=0.170,P值均0.05);女生青春期發(fā)育啟動(dòng)年齡與身高、體重、腰圍、體脂之間相關(guān)(r=0.809、r=0.623、r=0.262、r=-0.254,P值均0.05)。9、10、11、12歲組男生青春期發(fā)育啟動(dòng)者身高大于未啟動(dòng)者間;9、10、12歲組男生青春期發(fā)育啟動(dòng)者體重大于未啟動(dòng)者。同年齡組女孩青春期發(fā)育啟動(dòng)組的身高、體重、腰圍、上臂部皮褶厚度、肩胛部皮褶厚度、腹部皮褶厚度的均值均大于未啟動(dòng)組(P值均0.05)。4.青春期發(fā)育與體力活動(dòng)、接觸家用化學(xué)品等因素分析三次調(diào)查男生體力活動(dòng)得分的均值分別為12.6±5.9、13.8±6.2、14.6±6.2;女生體力活動(dòng)得分均值分別為11.1±5.0、10.7±4.6、11.5±4.8。三次調(diào)查男生日常接觸家用化學(xué)品的得分均值分別為2.7±1.9、2.2±1.7、2.2±2.5;女生日常接觸家用化學(xué)品的得分均值分別為2.7±2.0、2.5±1.8、2.6±2.2。三次調(diào)查男生接觸食品包裝材料的得分分別為0.8±1.0、0.8±1.0、0.7±0.9;女生接觸食品包裝材料的得分情況分別為0.8±1.0、0.7±1.0、0.9±1.0。三次調(diào)查男生心理行為發(fā)育困難總分分別為7.9±1.8、7.8±1.9、7.9±2.0,親社會(huì)行為得分分別為9.9±4.8、16.4±4.1、16.5±4.1;女生心理行為發(fā)育困難總分分別為7.2±2.0、7.5±2.0、7.4±2.2,親社會(huì)行為得分分別為 17.5±4.0、16.9±4.5、16.9±4.9。男女生的體力活動(dòng)得分、日常接觸家用化學(xué)品得分、食用食物的食品包裝材料得分、晨尿中環(huán)境內(nèi)分泌干擾物、心理行為發(fā)育等指標(biāo)與男女生的青春期發(fā)育啟動(dòng)年齡、各年齡男女生青春期發(fā)育啟動(dòng)及未啟動(dòng)間未見(jiàn)相關(guān)性。結(jié)論1.調(diào)查期間,同一年齡女孩,乳房發(fā)育率高于來(lái)潮率;谡{(diào)查時(shí),男孩青春發(fā)動(dòng)提前率高于女孩,隨訪結(jié)束時(shí),女孩青春發(fā)動(dòng)提前率高于男孩。2.同一年齡女孩,青春期發(fā)育啟動(dòng)者的身高、體重、腰圍、上臂部皮褶厚度、肩胛部皮褶厚度、腹部皮褶厚度均比青春期發(fā)育未啟動(dòng)者大。3.男女生的體力活動(dòng)、日常接觸家用化學(xué)品、食物包裝材料、晨尿中環(huán)境內(nèi)分泌干擾物、心理行為發(fā)育等指標(biāo)與青春發(fā)動(dòng)時(shí)相與間未發(fā)現(xiàn)相關(guān)關(guān)系。
[Abstract]:Objective To observe and observe the status quo of young students' youth phase and the possible influencing factors of early initiation of youth initiation in Zhengzhou, and to explore the possible gender differences in the influence of these factors on youth initiation, and to provide a basis for revealing the mechanism of the impact of youth starting phase.
