接觸噪聲、高溫和倒班與男性鋼鐵工人正常高值血壓發(fā)生和高血壓患病的關(guān)系

【摘要】:[目的]了解男性鋼鐵工人正常高值血壓、高血壓檢出現(xiàn)狀,分析接觸噪聲、高溫和倒班對(duì)男性鋼鐵工人正常高值血壓發(fā)生和高血壓患病的影響。[方法]以唐山某鋼鐵集團(tuán)2015年9月—2016年7月進(jìn)行職業(yè)健康檢查的全部男性在職職工為調(diào)查對(duì)象,進(jìn)行問(wèn)卷調(diào)查和體格檢查,采用多因素?zé)o序多分類(lèi)logistic回歸模型,分析接觸噪聲、高溫和倒班對(duì)男性鋼鐵工人正常高值血壓發(fā)生和高血壓患病的影響。[結(jié)果]本次調(diào)查共納入研究對(duì)象7 587人,男性鋼鐵工人正常高值血壓檢出率為42.9%,高血壓患病率為32.8%。接觸噪聲、高溫和倒班的男性鋼鐵工人血壓分布不全相同(P0.001)。調(diào)整年齡、工齡、民族、婚姻狀況、文化程度、家庭人均月收入、體質(zhì)量指數(shù)、高血壓家族史、吸煙、飲酒、食鹽程度、體育鍛煉等因素后,多因素?zé)o序多分類(lèi)logistic回歸分析結(jié)果顯示:分別以從不接觸噪聲、高溫和倒班的男性鋼鐵工人為參照,現(xiàn)在接觸噪聲、高溫和倒班的男性鋼鐵工人正常高值血壓發(fā)生風(fēng)險(xiǎn),即OR及其95%CI值分別為從不接觸者的1.27(1.12~1.44)倍、1.22(1.07~1.39)倍、1.20(1.03~1.40)倍,高血壓患病風(fēng)險(xiǎn)(OR及其95%CI)分別為從不接觸者的1.29(1.12~1.49)倍、1.34(1.16~1.56)倍和1.28(1.08~1.53)倍;曾經(jīng)倒班的男性鋼鐵工人正常高值血壓發(fā)生、高血壓患病風(fēng)險(xiǎn)分別是從不倒班者的1.26(1.03~1.55)倍和1.36(1.09~1.71)倍;曾經(jīng)接觸高溫的男性鋼鐵工人高血壓患病風(fēng)險(xiǎn)是從不接觸者的1.87(1.37~2.56)倍;曾經(jīng)接觸噪聲者正常高值血壓發(fā)生風(fēng)險(xiǎn)是從不接觸者的1.56(1.11~2.20)倍。[結(jié)論]接觸噪聲、高溫和倒班的男性鋼鐵工人正常高值血壓的發(fā)生和高血壓患病風(fēng)險(xiǎn)均高于不接觸者。
[Abstract]:[Objective] to investigate the normal high blood pressure and the detection of hypertension in male steel workers, and to analyze the effects of exposure noise, high temperature and shift shift on the occurrence of normal high blood pressure and the prevalence of hypertension in male steel workers. [Methods] A questionnaire survey and physical examination were conducted among all the male in-service workers in Tangshan Iron and Steel Group from September 2015 to July 2016. The effects of exposure noise, high temperature and shift shift on the occurrence of normal high blood pressure and hypertension in male steel workers were analyzed by multivariate disordered multiple classification logistic regression model. [Results: a total of 7 587 subjects were included in this study. The positive rate of normal high blood pressure in male steel workers was 42.9, and the prevalence of hypertension was 32.8. The blood pressure distribution of male steel workers with high temperature and shift work was not all the same (P 0.001). Adjusted age, length of service, nationality, marital status, education level, monthly income per family, body mass index. After family history of hypertension, smoking, drinking, salt degree, physical exercise and other factors, multivariate disorder multiple classification logistic regression analysis showed that: no exposure to noise. High temperature and shift male steel workers are now exposed to noise, heat and shift male steel workers for normal high blood pressure risk. The OR and 95 CI values were 1.27 / 1.121.44) times and 1.22 / 1.07 / 1.39 times of those of the non-contact persons, respectively. The risk of hypertension and its 95 CI were 1.29 / 1.121.49) times of those who had never been in contact with each other. 1.34, 1.16, 1.56) and 1.28, 1.08, 1.53) times; The normal high blood pressure occurred in the male steel workers who had been working shift, the risk of hypertension was 1.261.03 (1.55) times and 1.71times (1.361.09 / 1.71), respectively. Male steel workers exposed to high temperature had a higher risk of hypertension than those who had never been exposed to high temperature, and the risk of hypertension was 2.56 times higher than that of those who had never been exposed to high temperature. The risk of normal high blood pressure in those who had been exposed to noise was 1.56 ~ 1.11 ~ 2.20 times higher than that in those who had never been exposed to noise. [Conclusion: the incidence of normal high blood pressure and the risk of hypertension in male steel workers exposed to noise, high temperature and shift shift were higher than those of non exposed workers.
