【摘要】：目的:探討我院事前審方系統(tǒng)的建立及開展對(duì)門診合理用藥的影響。方法:根據(jù)門診藥房實(shí)際需求,對(duì)原有的醫(yī)院信息系統(tǒng)(HIS)相應(yīng)模塊進(jìn)行修改后建立藥房事前審方系統(tǒng),將該系統(tǒng)用于繳費(fèi)前審核門診處方并實(shí)施相應(yīng)的配套管理措施。比較事前審方系統(tǒng)運(yùn)行前(2014年4-9月)、后(2015年4-9月)各相關(guān)評(píng)價(jià)指標(biāo)的變化。結(jié)果:通過(guò)在HIS中按相關(guān)標(biāo)準(zhǔn)建立審核處方模板、制訂審方系統(tǒng)工作流程及進(jìn)行處方干預(yù)管理,我院實(shí)現(xiàn)了門診處方在繳費(fèi)前的處方合理性審核。與運(yùn)行前比較,運(yùn)行后口服類抗菌藥物使用率、注射類抗菌藥物使用率、不合理處方率分別由25.7%、31.5%、5.10%下降至16.5%、19.8%、1.98%;不合理處方干預(yù)成功率由24.3%上升至85.5%;錯(cuò)誤處方不良反應(yīng)發(fā)生率、醫(yī)保費(fèi)用超標(biāo)率和門診患者投訴率均顯著降低(P0.000 1)。結(jié)論:事前審方系統(tǒng)的建立可促進(jìn)醫(yī)院合理用藥及服務(wù)水平的提升。
[Abstract]:Objective: to explore the establishment of pre-trial system in our hospital and its influence on rational use of drugs in out-patient department. Methods: according to the actual demand of outpatient pharmacy, the corresponding module of the original hospital information system (HIS) was modified and the pre-trial system of pharmacy was established. The system was used to examine the outpatient prescriptions before payment and implement the corresponding supporting management measures. The changes of the evaluation indexes before the operation of the pre-trial system (April-September 2014) and after the operation (April-September 2015) were compared. Results: through establishing the audit prescription template according to the relevant standards in HIS, drawing up the workflow of the audit system and carrying on the prescription intervention management, our hospital realized the rational audit of the prescription before payment of the prescription in our hospital. Compared with pre-operation, the utilization rate of oral antibiotics and injection antibiotics decreased from 25.7%, 31.5%, 5.10% to 16.5%, 19.8% and 1.98% respectively after operation, and the irrational prescription rates decreased from 25.7%, 31.5%, 5.10% to 16.5%, 19.8% and 1.98%, respectively. The success rate of unreasonable prescription intervention increased from 24.3% to 85.5%, and the incidence of adverse reactions of wrong prescriptions, the excess rate of medical insurance costs and the rate of complaints of outpatients were significantly decreased (P0.000 1). Conclusion: the establishment of pre-trial system can promote the rational use of drugs and the improvement of service level in hospitals.
