【摘要】：目的:為制定促進(jìn)基本藥物在二、三級(jí)醫(yī)療衛(wèi)生機(jī)構(gòu)推廣應(yīng)用的政策提供依據(jù)。方法:調(diào)取2012年1月-2015年12月南京市屬所有的共計(jì)31家二、三級(jí)醫(yī)療衛(wèi)生機(jī)構(gòu)的藥品采購入庫數(shù)據(jù),利用Excel 2010和SPSS 20.0軟件匯總、統(tǒng)計(jì)和分析基本藥物的采購金額和配備數(shù)量情況。結(jié)果:2012-2015年,31家醫(yī)療衛(wèi)生機(jī)構(gòu)基本藥物采購金額占藥品總采購金額的比例(簡稱"基本藥物采購金額占比")平均分別為18.33%、18.56%、17.19%、17.53%,國家基本藥物采購金額占基本藥物采購金額的比例(簡稱"國家基本藥物采購金額占比")平均分別為31.14%、29.40%、25.69%、25.79%,配備的基本藥物通用名數(shù)平均分別為189.84、205.58、210.26、206.65個(gè);其中,2014年基本藥物采購金額占比、國家基本藥物采購金額占比均較2013年顯著降低(P0.05),2013年配備的基本藥物通用名數(shù)較2012年顯著增加(P0.05)。各年度二級(jí)醫(yī)療衛(wèi)生機(jī)構(gòu)基本藥物采購金額占比均顯著高于三級(jí)醫(yī)療衛(wèi)生機(jī)構(gòu);綜合醫(yī)院基本藥物采購金額占比均顯著高于�？漆t(yī)院,配備的基本藥物通用名數(shù)均顯著高于中醫(yī)院和�？漆t(yī)院;下轄社區(qū)衛(wèi)生服務(wù)中心的二級(jí)醫(yī)療衛(wèi)生機(jī)構(gòu)基本藥物采購金額占比均顯著高于未下轄社區(qū)衛(wèi)生服務(wù)中心的二級(jí)醫(yī)療衛(wèi)生機(jī)構(gòu),差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。各年度綜合醫(yī)院基本藥物采購金額占比與藥品總采購金額均具有負(fù)相關(guān)性(P0.05);所有醫(yī)療衛(wèi)生機(jī)構(gòu)配備的基本藥物通用名數(shù)與全部藥品通用名數(shù)均具有正相關(guān)性(P0.05)。結(jié)論:2012-2015年南京市二、三級(jí)醫(yī)療衛(wèi)生機(jī)構(gòu)基本藥物采購金額占比、國家基本藥物采購金額占比及配備的基本藥物通用名數(shù)差異較大,數(shù)值普遍較低,除個(gè)別醫(yī)療衛(wèi)生機(jī)構(gòu)外絕大多數(shù)沒有達(dá)到江蘇省衛(wèi)生計(jì)生委的要求。醫(yī)療衛(wèi)生機(jī)構(gòu)的級(jí)別、類型、藥品采購配備規(guī)模及是否下轄社區(qū)衛(wèi)生服務(wù)中心均可能對(duì)其基本藥物采購配備產(chǎn)生影響。
[Abstract]:Objective: to provide the basis for the policy of promoting the application of essential drugs in the secondary and tertiary medical and health institutions. Methods: from January 2012 to December 2015, the data of drug purchase from 31 second and third level medical and health institutions in Nanjing were collected and collected by Excel 2010 and SPSS 20.0 software. Statistics and analysis of the purchase amount and quantity of essential drugs. Results: from 2012 to 2015, the proportion of essential drugs purchased from 31 medical and health institutions to the total amount of drugs purchased (referred to as "the proportion of essential drug purchases") was 18.33 and 18.566.19 respectively. The proportion of the amount of essential drugs purchased by the State to the amount of essential drugs purchased (referred to as "the proportion of the amount of essential drugs purchased by the State") is 31.14% and 29.40% respectively. The average number of generic names of essential drugs was 189.84205.58210.26206.65; Among them, the proportion of essential drugs purchase amount in 2014 was significantly lower than that in 2013 (P0.05), and the number of general names of essential drugs in 2013 significantly increased compared with 2012 (P0.05). The proportion of the purchase amount of essential drugs in the second-level medical and health institutions was significantly higher than that in the third-level medical and health institutions. The proportion of purchase amount of essential drugs in general hospitals was significantly higher than that in specialized hospitals, and the number of generic names of essential drugs in general hospitals was significantly higher than that in Chinese medicine hospitals and specialized hospitals. The proportion of the purchase amount of essential drugs in the second level medical and health institutions under the jurisdiction of the community health service center was significantly higher than that in the second level of the community health service center without jurisdiction, and the difference was statistically significant (P0.05). There was a negative correlation between the proportion of purchase amount of essential drugs in general hospitals and the total purchase amount of drugs (P0.05). There was a positive correlation between the number of generic names of essential drugs and the number of generic names of all medicines in all medical and health institutions (P0.05). Conclusion: from 2012 to 2015, the proportion of purchase amount of essential drugs in the second and third level medical and health institutions in Nanjing City was higher, and the proportion of national essential drugs purchase amount and the number of general names of the essential drugs provided were quite different, and the values were generally lower. In addition to individual medical and health institutions, the vast majority did not meet the requirements of the Jiangsu Provincial Health and Family Planning Commission. The level and type of medical and health institutions, the size of drug procurement equipment and whether the community health service centers are under the jurisdiction may have an impact on the procurement of essential drugs.
