我國現行藥品醫(yī)保談判機制運行情況調查研究

【摘要】:目的:為我國藥品醫(yī)保談判機制的進一步完善提供參考。方法:對參與江蘇省、江西省、青島市、成都市四地藥品醫(yī)保談判的部分醫(yī)保經辦機構和醫(yī)藥企業(yè)工作人員進行問卷調查,了解其業(yè)務熟悉情況和對談判機制的認可程度,并采用描述性分析、獨立樣本t檢驗分析等方法對數據進行統(tǒng)計和分析。結果:共發(fā)放問卷70份,回收有效問卷68份,有效回收率為97.14%;包括醫(yī)保經辦機構受訪者21人,醫(yī)藥企業(yè)受訪者47人。對于業(yè)務熟悉情況,受訪者各方面評分平均值均3(4分制),以"藥品談判項目執(zhí)行過程中的管理方式"評分最低;兩類受訪者在"藥品談判過程應遵循的基本原則""實質性談判過程中的談判目標""業(yè)務熟悉情況總評分"方面的評分比較差異均有統(tǒng)計學意義(P0.05),醫(yī)藥企業(yè)受訪者均高于醫(yī)保經辦機構受訪者。在對談判機制的認可程度上,對談判機制整體效果的認可程度除"有利于維持藥企的價格體系"外其余各方面評分平均值均≥4(5分制),對談判準備工作、實質性談判工作、談判協議執(zhí)行情況、談判項目監(jiān)管評估情況的認可程度除"患者取藥用藥方便快捷"外其他各環(huán)節(jié)各方面評分平均值均4。兩類受訪者對談判機制整體效果、談判準備工作、談判項目監(jiān)管評估情況的認可程度各方面評分比較差異均無統(tǒng)計學意義(P0.05);而對實質性談判工作的認可程度在"談判過程中雙方在平等、合作、融洽的氛圍下進行談判"方面的評分比較差異有統(tǒng)計學意義(P0.05),醫(yī)保經辦機構受訪者高于醫(yī)藥企業(yè)受訪者;同時,對談判協議執(zhí)行情況的認可程度在"在協議生效后,醫(yī)保經辦機構嚴格按照協議執(zhí)行""基金向藥企方支付及時、方式合理"兩方面的評分比較差異均有統(tǒng)計學意義(P≤0.05),醫(yī)保經辦機構受訪者均高于醫(yī)藥企業(yè)受訪者。結論:現行藥品醫(yī)保談判機制整體效果較好,運行較為順暢,但仍存在準備階段收集的依據材料不夠充分、患者取藥用藥流程繁復、談判雙方地位不平等、基金向藥企方支付不及時、評估工作不完善等不足。
[Abstract]:Objective: to provide a reference for the further improvement of the negotiation mechanism of drug and medical insurance in China. Methods: a questionnaire survey was conducted among some medical insurance agencies and staff of pharmaceutical enterprises in Jiangsu, Jiangxi, Qingdao and Chengdu to find out their business familiarity and the degree of approval of the negotiation mechanism. Descriptive analysis, independent sample t-test analysis and other methods are used to analyze the data. Results: a total of 70 questionnaires were sent out and 68 valid questionnaires were collected, with an effective recovery rate of 97.14, including 21 respondents from medical insurance agencies and 47 respondents from pharmaceutical enterprises. For business familiarity, the average score of all aspects of the interviewees was 3 (4 points system), with the lowest score of "the management mode in the execution of drug negotiation project"; There were significant differences between the two groups of respondents in terms of "the basic principles to be followed in the Drug negotiation process", "the negotiation objectives in the substantive negotiation process" and "the General score of Business familiarity" (P0.05), while the respondents of pharmaceutical enterprises had significant differences (P0.05). They were higher than the respondents of medical insurance agencies.
