【摘要】：目的:探討責(zé)任藥師在靜脈藥物配置中心(PIVAS)不合理醫(yī)囑干預(yù)中的作用,促進(jìn)臨床合理用藥。方法:回顧性分析我院PIVAS 2012年1-6月501 115份醫(yī)囑及建立責(zé)任藥師制度并進(jìn)行干預(yù)后的2013年7-12月499 189份醫(yī)囑,對不合理醫(yī)囑進(jìn)行整理分類和歸納分析。結(jié)果:干預(yù)前不合理醫(yī)囑共918份(0.183 2%),干預(yù)后不合理醫(yī)囑下降為448份(0.089 7%)。其中,溶劑選擇不合理醫(yī)囑由134份(0.026 7%)下降為69份(0.013 8%),溶劑體積不合理醫(yī)囑由435份(0.086 8%)下降為206份(0.041 3%),給藥劑量不合理醫(yī)囑由241份(0.048 1%)下降為117份(0.023 4%),配伍不合理醫(yī)囑由51份(0.010 2%)下降為28份(0.005 6%),給藥頻次不合理醫(yī)囑由17份(0.003 4%)下降為2份(0.000 4%)。結(jié)論:責(zé)任藥師干預(yù)靜脈用藥不合理醫(yī)囑效果明顯,提高了臨床合理用藥水平,保障了患者用藥安全。
[Abstract]:Objective: to explore the role of responsible pharmacists in the intervention of unreasonable doctor's orders in intravenous Drug allocation Center (IVAS). Methods: a retrospective analysis of 501,115 medical orders and 499,189 medical orders from July to December 2013 in PIVAS of our hospital from January to June, 2012, and the establishment of a responsible pharmacist system were carried out to promote rational use of drugs in clinical practice. Results: a total of 918 unreasonable medical orders were collected and analyzed before intervention, and after intervention, the number of unreasonable orders decreased to 448 / 0.089 / 7. among them, the number of unreasonable medical orders before intervention was 0.183 / 0.183 / 0.183 / 0.183 / 0.183 respectively, and the number of unreasonable medical orders decreased to 448 / 0.089 / 7a after intervention. The number of unreasonable choice of solvent decreased from 134 to 0.026 772) to 69 to 0.013 8, the volume of solvent to 206 from 435 to 0.0868, and the amount of medication to 117 from 241 to 0.023 4. From 51 (0.010 2) to 28 (0.005 6), the frequency of administration was decreased from 17 (0.003 4) to 2 (0.000 4). Conclusion: the effect of intervention by responsible pharmacists on irrational prescriptions of intravenous medication is obvious. It improves the level of rational drug use in clinic and ensures the safety of drug use in patients.
