【摘要】：20世紀(jì)50年代至今,醫(yī)療機(jī)構(gòu)制劑發(fā)展經(jīng)歷漫長(zhǎng)的變化和改革,從產(chǎn)生、發(fā)展到萎縮,展示我國(guó)醫(yī)療機(jī)構(gòu)制劑行業(yè)發(fā)展的整體態(tài)勢(shì)。醫(yī)療機(jī)構(gòu)制劑作為市售藥品的補(bǔ)充,具有安全有效、價(jià)格低廉等特點(diǎn),在疾病治療方面發(fā)揮不可替代的作用。在國(guó)家政策要求以及社會(huì)關(guān)注下,監(jiān)管部門(mén)對(duì)醫(yī)療機(jī)構(gòu)制劑嚴(yán)格控制,確保使用安全有效,這將使醫(yī)療機(jī)構(gòu)制劑呈現(xiàn)出新的發(fā)展特點(diǎn)。本研究采用文獻(xiàn)梳理、問(wèn)卷調(diào)查和實(shí)地走訪(fǎng)方式對(duì)北京市具有制劑批準(zhǔn)文號(hào)的醫(yī)療機(jī)構(gòu)開(kāi)展調(diào)研。本次調(diào)研覆蓋130家具有制劑批準(zhǔn)文號(hào)的醫(yī)療機(jī)構(gòu),占全市具有制劑批準(zhǔn)文號(hào)的醫(yī)療機(jī)構(gòu)93.5%。共回收問(wèn)卷數(shù)量117份,回收率達(dá)到90%。調(diào)研出北京市具有批準(zhǔn)文號(hào)制劑品種2317個(gè),占全市總品種的67.2%。本研究在闡述北京市醫(yī)療機(jī)構(gòu)制劑整體現(xiàn)狀基礎(chǔ)上,對(duì)醫(yī)療機(jī)構(gòu)制劑配制、研發(fā)及監(jiān)管現(xiàn)狀開(kāi)展定性和定量研究。首先,針對(duì)北京市醫(yī)療機(jī)構(gòu)制劑整體現(xiàn)狀開(kāi)展調(diào)查研究,以全市具有制劑批準(zhǔn)文號(hào)品種的醫(yī)療機(jī)構(gòu)作為研究對(duì)象,了解醫(yī)院性質(zhì)、醫(yī)院級(jí)別以及經(jīng)濟(jì)效益、配制條件等材料；其次,針對(duì)具有批準(zhǔn)文號(hào)制劑品種的配制情況、生產(chǎn)情況以及科室分布情況等進(jìn)行定量研究；再次,針對(duì)北京市醫(yī)療機(jī)構(gòu)制劑研制開(kāi)發(fā)狀況,深入挖掘影響醫(yī)療機(jī)構(gòu)制劑研發(fā)的因素；最后,通過(guò)SWOT-PEST分析得出北京市醫(yī)療機(jī)構(gòu)制劑發(fā)展存在問(wèn)題,并提出相應(yīng)解決方案及對(duì)策。通過(guò)本研究得出北京市醫(yī)療機(jī)構(gòu)制劑現(xiàn)狀概括如下：醫(yī)療機(jī)構(gòu)制劑發(fā)展呈現(xiàn)萎縮態(tài)勢(shì),批準(zhǔn)文號(hào)數(shù)量以及許可證數(shù)量逐年下滑；醫(yī)療機(jī)構(gòu)制劑軟硬件投入不足,制劑室設(shè)備條件較差,人員數(shù)量偏低,專(zhuān)業(yè)素質(zhì)不高；醫(yī)療機(jī)構(gòu)缺乏制劑研發(fā)動(dòng)力,積極性不高；醫(yī)療機(jī)構(gòu)制劑發(fā)展受經(jīng)濟(jì)效益影響較大,多數(shù)制劑存在價(jià)格與成本倒掛現(xiàn)象；醫(yī)療機(jī)構(gòu)制劑發(fā)展得到監(jiān)管部門(mén)的支持,在制劑質(zhì)量和安全性方面均有提高；醫(yī)療機(jī)構(gòu)制劑監(jiān)管法律法規(guī)體系初步形成,需要進(jìn)一步落實(shí)完善。本研究從監(jiān)管層面、醫(yī)療機(jī)構(gòu)層面和社會(huì)層面分別提出相關(guān)政策建議如下：醫(yī)療機(jī)構(gòu)制劑監(jiān)管部門(mén)需要加大監(jiān)管力度,嚴(yán)格把控制劑配制、審批及使用等多個(gè)環(huán)節(jié),充分整合資源為制劑發(fā)展提供支持；醫(yī)療機(jī)構(gòu)要重視制劑發(fā)展,加大制劑科研投入,為新藥研發(fā)提供基礎(chǔ)；從社會(huì)角度建立制劑區(qū)域配制中心,利于節(jié)約資源,形成規(guī)模效益。
[Abstract]:Since 1950s, the development of medical preparations after a long change and reform, from development to production, atrophy, show preparation industry development of medical institutions in China. The overall trend of medical preparations as commercially available drugs, is safe and effective, low price and other characteristics, play an irreplaceable role in disease treatment. The requirements of national policy and social attention, supervision departments to strictly control the preparation of medical institutions, to ensure that the use of safe and effective, which will make medical preparations showing new features. This study adopts literature review, questionnaire survey and field visits to medical institutions have preparation approval number in Beijing City, the medical institutions to carry out research. The research covered 130 preparations with the approval number, accounting for the city's preparations with approval of the medical institutions 93.5%. questionnaires the number 117, the recovery rate of Research in Beijing city has achieved 90%. approval preparation varieties 2317, representing the city's total varieties of 67.2%. this study in Beijing city based on the current situation of the whole medical preparation, preparation of medical preparation, qualitative and quantitative research to carry out research and supervision. Firstly, the needle to investigate the overall status of the preparation of medical institutions in Beijing city in the city, with preparation approval number varieties of medical institutions as the research object, understand the nature of hospital, hospital level and economic benefits, preparation conditions and other materials; secondly, according to the situation of preparation of approval preparations, quantitative research on production and Distribution Department; thirdly, according to the medical institutions of Beijing city development situation of preparation dig, factors influence medical institutions pharmaceutical research and development; finally, through the SWOT-PEST analysis of Beijing City medical institutions preparations The development of existing problems, and puts forward the corresponding solutions and countermeasures. Through the research of Beijing City medical preparation status are summarized as follows: formulation development of medical institutions is shrinking, the approval number and license number number decreased year by year; the shortage of medical preparations of hardware and software, equipment preparation condition is poor, the number of personnel is low, professional quality high; medical institutions lack of preparation research motivation, enthusiasm is not high; the preparation of medical institutions by the economic development influence, most formulations of the price and cost inversion phenomenon; medical institutions pharmaceutical development regulatory support in the preparation of quality and safety of medical institutions were improved; laws and regulations formulation supervision system initially formed. To further improve the implementation. This study from the perspective of supervision, medical institution level and the social level put forward some relevant policy Suggestions are as follows: the supervision department of medical preparations in medical institutions need to strengthen supervision, strictly control preparation, approval and use of multiple links, to fully integrate resources to provide support for the preparation of development; medical institutions should pay attention to pharmaceutical development, increase the preparation of investment in scientific research, provide the basis for new drug research and development; establish the preparation area preparing center from the angle of social.
