【摘要】：20世紀50年代至今,醫(yī)療機構(gòu)制劑發(fā)展經(jīng)歷漫長的變化和改革,從產(chǎn)生、發(fā)展到萎縮,展示我國醫(yī)療機構(gòu)制劑行業(yè)發(fā)展的整體態(tài)勢。醫(yī)療機構(gòu)制劑作為市售藥品的補充,具有安全有效、價格低廉等特點,在疾病治療方面發(fā)揮不可替代的作用。在國家政策要求以及社會關(guān)注下,監(jiān)管部門對醫(yī)療機構(gòu)制劑嚴格控制,確保使用安全有效,這將使醫(yī)療機構(gòu)制劑呈現(xiàn)出新的發(fā)展特點。本研究采用文獻梳理、問卷調(diào)查和實地走訪方式對北京市具有制劑批準文號的醫(yī)療機構(gòu)開展調(diào)研。本次調(diào)研覆蓋130家具有制劑批準文號的醫(yī)療機構(gòu),占全市具有制劑批準文號的醫(yī)療機構(gòu)93.5%。共回收問卷數(shù)量117份,回收率達到90%。調(diào)研出北京市具有批準文號制劑品種2317個,占全市總品種的67.2%。本研究在闡述北京市醫(yī)療機構(gòu)制劑整體現(xiàn)狀基礎(chǔ)上,對醫(yī)療機構(gòu)制劑配制、研發(fā)及監(jiān)管現(xiàn)狀開展定性和定量研究。首先,針對北京市醫(yī)療機構(gòu)制劑整體現(xiàn)狀開展調(diào)查研究,以全市具有制劑批準文號品種的醫(yī)療機構(gòu)作為研究對象,了解醫(yī)院性質(zhì)、醫(yī)院級別以及經(jīng)濟效益、配制條件等材料；其次,針對具有批準文號制劑品種的配制情況、生產(chǎn)情況以及科室分布情況等進行定量研究；再次,針對北京市醫(yī)療機構(gòu)制劑研制開發(fā)狀況,深入挖掘影響醫(yī)療機構(gòu)制劑研發(fā)的因素；最后,通過SWOT-PEST分析得出北京市醫(yī)療機構(gòu)制劑發(fā)展存在問題,并提出相應(yīng)解決方案及對策。通過本研究得出北京市醫(yī)療機構(gòu)制劑現(xiàn)狀概括如下：醫(yī)療機構(gòu)制劑發(fā)展呈現(xiàn)萎縮態(tài)勢,批準文號數(shù)量以及許可證數(shù)量逐年下滑；醫(yī)療機構(gòu)制劑軟硬件投入不足,制劑室設(shè)備條件較差,人員數(shù)量偏低,專業(yè)素質(zhì)不高；醫(yī)療機構(gòu)缺乏制劑研發(fā)動力,積極性不高；醫(yī)療機構(gòu)制劑發(fā)展受經(jīng)濟效益影響較大,多數(shù)制劑存在價格與成本倒掛現(xiàn)象；醫(yī)療機構(gòu)制劑發(fā)展得到監(jiān)管部門的支持,在制劑質(zhì)量和安全性方面均有提高；醫(yī)療機構(gòu)制劑監(jiān)管法律法規(guī)體系初步形成,需要進一步落實完善。本研究從監(jiān)管層面、醫(yī)療機構(gòu)層面和社會層面分別提出相關(guān)政策建議如下：醫(yī)療機構(gòu)制劑監(jiān)管部門需要加大監(jiān)管力度,嚴格把控制劑配制、審批及使用等多個環(huán)節(jié),充分整合資源為制劑發(fā)展提供支持；醫(yī)療機構(gòu)要重視制劑發(fā)展,加大制劑科研投入,為新藥研發(fā)提供基礎(chǔ)；從社會角度建立制劑區(qū)域配制中心,利于節(jié)約資源,形成規(guī)模效益。
[Abstract]:Since 1950s, the development of medical institutions has undergone a long period of changes and reforms, from production, development to atrophy. To show the overall situation of the development of the pharmaceutical industry of medical institutions in China. As a supplement of the drugs sold in the market, the preparations of medical institutions have the characteristics of safety, efficiency, low price and so on. In the treatment of diseases play an irreplaceable role. In the national policy requirements and social concern, the regulatory authorities of medical institutions to strictly control the preparation, to ensure the use of safe and effective. This will make the preparation of medical institutions present new characteristics of development. Questionnaires and field visits to Beijing with the preparation approval number of medical institutions to carry out research. This survey covers 130 medical institutions with preparation approval number. Accounting for the whole city with the preparation approval number of medical institutions 93.510. A total of 117 questionnaires were recovered, the recovery rate reached 90. The survey shows that Beijing has 2317 kinds of approved preparation. This study on the basis of explaining the overall status of the preparation of medical institutions in Beijing, the preparation of medical institutions, research and development and regulatory status of qualitative and quantitative research. First of all. According to the status quo of the preparation of Beijing medical institutions, the authors investigated and studied the status quo of the preparations in the whole city, and took the medical institutions with the preparation approval number as the research object to understand the nature of the hospital, the hospital level and the economic benefits. Preparation conditions and other materials; Secondly, quantitative research is carried out on the preparation, production and distribution of approved preparations. Thirdly, according to the status of research and development of preparations in Beijing medical institutions, the factors that affect the research and development of preparations in medical institutions are deeply explored. Finally, through SWOT-PEST analysis, the development of medical institutions in Beijing has problems. And put forward the corresponding solutions and countermeasures. Through this study, the status quo of preparation in Beijing medical institutions is summarized as follows: the development of medical institutions is shrinking, the number of approved document number and the number of licenses decline year by year; The medical organization preparation hardware and software investment is insufficient, the preparation room equipment condition is poor, the personnel quantity is on the low side, the specialized quality is not high; Medical institutions lack the motivation of research and development of preparations, and their enthusiasm is not high; The development of preparation in medical institutions is greatly affected by economic benefits, and most of the preparations have the phenomenon of price and cost upside down. The development of preparation in medical institutions has been supported by the supervision department, and the quality and safety of preparations have been improved. The legal and regulatory system of pharmaceutical preparation regulation in medical institutions is initially formed and needs to be further improved. This study is from the level of supervision. Medical institutions and social level respectively put forward the relevant policy recommendations as follows: the pharmaceutical regulatory authorities of medical institutions need to strengthen supervision, strictly control the preparation of agents, examination and approval and use of a number of links. Fully integrate resources to provide support for formulation development; Medical institutions should attach importance to the development of preparations, increase the investment in research and development of new drugs, and provide the basis for the research and development of new drugs. The establishment of regional preparation center from a social point of view is beneficial to the conservation of resources and the formation of economies of scale.
