【摘要】：目的:探討鼻內(nèi)鏡術(shù)后納吸棉聯(lián)合布地奈德混懸液填塞術(shù)腔的臨床效果。方法:隨機(jī)選取30例行雙側(cè)鼻竇內(nèi)鏡手術(shù)且完成隨訪的CRSwNP和CRSsNP患者,分為實(shí)驗(yàn)組(左側(cè)鼻腔)和對(duì)照組(右側(cè)鼻腔)。實(shí)驗(yàn)組在手術(shù)結(jié)束時(shí)用含有布地奈德混懸液的納吸棉填塞術(shù)腔,對(duì)照組則單純使用納吸棉填塞術(shù)腔,分別在術(shù)后第2周、第1個(gè)月、第3個(gè)月時(shí)對(duì)患者行VAS評(píng)分及Lund-Kennedy內(nèi)鏡黏膜形態(tài)評(píng)分。結(jié)果:12組術(shù)后第2周、第1、3個(gè)月時(shí)患者整體不適感及鼻塞、鼻腔分泌物、頭痛或頭昏、鼻面部脹痛四大主觀癥狀的VAS評(píng)分差異均有統(tǒng)計(jì)學(xué)意義(P0.05),實(shí)驗(yàn)組優(yōu)于對(duì)照組。22組術(shù)后第2周、第1、3個(gè)月Lund-Kennedy內(nèi)鏡黏膜形態(tài)評(píng)分各項(xiàng)目得分及總分差異均有統(tǒng)計(jì)學(xué)意義(P0.05),實(shí)驗(yàn)組優(yōu)于對(duì)照組;術(shù)后第1、3個(gè)月時(shí),實(shí)驗(yàn)組的鼻腔阻力小于對(duì)照組(P0.05)。3術(shù)后第3個(gè)月,實(shí)驗(yàn)組28例(93.3%)病情完全控制,總有效率96.6%;對(duì)照組22例(73.3%)病情完全控制,總有效率93.3%。2組間病情完全控制率比較差異有統(tǒng)計(jì)學(xué)意義(P0.05);2組間總有效率比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:鼻內(nèi)鏡術(shù)后應(yīng)用含有布地奈德混懸液的納吸棉填塞術(shù)腔,可明顯改善患者術(shù)后的不適癥狀,促進(jìn)黏膜的順利轉(zhuǎn)歸,提高了病情完全控制率。
[Abstract]:Objective: to investigate the clinical effect of cotton aspiration combined with budesonide suspension filling cavity after endoscopic sinus surgery. Methods: 30 patients with CRSwNP and CRSsNP who underwent bilateral endoscopic sinus surgery were randomly divided into experimental group (left nasal cavity) and control group (right nasal cavity). At the end of the operation, the experimental group was filled with nalyl cotton with budesonide suspension, while the control group was treated with nalyl cotton tamponade alone. VAS score and Lund-Kennedy endoscopic mucous membrane morphology score were performed at the 2nd week, the first month and the third month after operation, respectively. Results: at the 2nd week, 1st and 3rd month after operation, there were significant differences in VAS scores of nasal congestion, nasal secretions, headache or dizziness and nasal and facial distension and pain in the 12 groups, and the experimental group was better than the control group. The scores and total scores of Lund-Kennedy endoscopic mucous membrane morphology scores in 22 groups were significantly different at the 2nd week, 1st and 3rd months after operation (P 0.05). The experimental group was superior to the control group. At the 1st and 3rd month after operation, the nasal resistance of the experimental group was lower than that of the control group (P 0.05). 3 at the 3rd month after operation, 28 cases (93.3%) in the experimental group were completely controlled, the total effective rate was 96.6%, 22 cases in the control group (73.3%) were completely controlled, and the total effective rate was 93.3%. There was no significant difference in the total effective rate between the two groups (P 0.05). Conclusion: the application of cotton suction tamponade with budesonide suspension after nasal endoscopy can obviously improve the uncomfortable symptoms, promote the smooth prognosis of mucous membrane and improve the complete control rate of the disease.
