聲帶溝患者最小聲門面積與嗓音聲學(xué)指標(biāo)的特征及相關(guān)性研究

【摘要】:目的探討聲帶溝患者發(fā)聲時最小聲門面積(minimum glottal area,MGA)和嗓音聲學(xué)指標(biāo)的特征及相關(guān)性。方法選擇60例正常人(正常組,男女各30例)和病理型聲帶溝患者62例(患者組,男33例,女29例)進(jìn)行頻閃喉鏡檢查,應(yīng)用配套KIPS軟件計算患者發(fā)/i:/音時的MGA;采用Dr.Speech聲學(xué)分析軟件測試其基頻(F0)、基頻微擾(jitter)、振幅微擾(shimmer)、標(biāo)準(zhǔn)化噪聲能量(normalized noise energy,NNE)、諧噪比(harmony to noise ratio,HNR)、信噪比(signal noise ratio,SNR)、最長聲時(maximum phonation time,MPT),并分別與發(fā)聲時的MGA進(jìn)行相關(guān)性分析。結(jié)果發(fā)/i:/音時,男性患者組MGA(434.74±112.83像素)明顯大于男性正常組(298.25±93.63像素)(P0.05);女性患者組MGA(484.75±143.91像素)明顯大于女性正常組(293.43±93.73像素)(P0.05);男性及女性患者發(fā)聲時MGA分別與基頻(r=0.972,P0.05;r=0.928,P0.05)、基頻微擾(r=0.978,P0.05;r=0.910,P0.05)、振幅微擾(r=0.973,P0.05;r=0.921,P0.05)、標(biāo)準(zhǔn)化噪聲能量(r=0.883,P0.05;r=0.960,P0.05)呈正相關(guān)關(guān)系,與信噪比(r=-0.947,P0.05;r=-0.957,P0.05)、諧噪比(r=-0.959,P0.05;r=-0.944,P0.05)、最長聲時(r=-0.891,P0.05;r=-0.936,P0.05)呈負(fù)相關(guān)關(guān)系。結(jié)論病理型聲帶溝患者發(fā)/i:/音時聲門面積較正常人增大,其MGA值越大嗓音的SNR、HNR及MPT值越小,而F0、jitter、shimmer、NNE則隨MGA值增大而增大。
[Abstract]:Objective to investigate the characteristics and correlation between the minimum glottic area (minimum glottal area,MGA) and voice acoustic indexes in patients with vocal cord groove. Methods 60 normal subjects (30 males and 30 males) and 62 patients with pathological vocal cord groove (33 males and 29 females) were examined by strobe laryngoscope. MGA; was calculated by matching KIPS software. The fundamental frequency (F0), fundamental frequency perturbation (jitter), amplitude perturbation (shimmer), standardized noise energy (normalized noise energy,NNE), harmonic noise ratio (harmony to noise ratio,HNR), signal-to-noise ratio (signal noise ratio,SNR) and maximum sound time (maximum phonation time,MPT) were measured by Dr.Speech acoustic analysis software, and the correlation with MGA was analyzed.
