LASIK術(shù)后眼調(diào)節(jié)幅度的改變及其與視疲勞的關(guān)系

【摘要】: 目的:準(zhǔn)分子激光角膜屈光手術(shù)在我國已有近二十年的發(fā)展歷程,它的出現(xiàn)為無數(shù)的屈光不正患者尤其是近視眼患者摘掉了沉重的眼鏡,在人們追求高效率工作與高質(zhì)量生活的今天,它也成為了很多追求時尚的戴鏡人群的一種可以信賴的選擇。但是在臨床工作中我們發(fā)現(xiàn)有部分近視患者在術(shù)后早期出現(xiàn)了近距離工作的視疲勞的癥狀,干擾了正常工作和學(xué)習(xí),這應(yīng)該引起我們的關(guān)注。本次研究主要是探討準(zhǔn)分子激光原位磨鑲術(shù)對近視患者術(shù)后單眼及雙眼調(diào)節(jié)幅度的影響,并對調(diào)節(jié)幅度的變化與視疲勞癥狀的關(guān)系進行探討,為LASIK術(shù)后視覺質(zhì)量的預(yù)測提供一定的臨床依據(jù)。 方法:入選對象為2009年3月至2009年8月來大連醫(yī)科大學(xué)附屬第二醫(yī)院眼科行LASIK治療的雙眼近視患者年齡在18-35歲之間,共60例120眼,按患者術(shù)前近視屈光度分為低中度近視組36例72眼(等效球鏡度≤-6.00D),高度近視組24例48眼(等效球鏡度-6.00D),正視組為20例40眼(年齡在25.34±5.37,等效球鏡度為-0.50-+0.50,BCVA≥1.0)。正視組與近視組分別在術(shù)前,術(shù)后1w,術(shù)后2W,術(shù)后1月測定其單眼及雙眼調(diào)節(jié)幅度。分別對正視組和近視組在各個時間對其進行近距工作視疲勞癥狀的問卷調(diào)查。并記錄數(shù)據(jù)。手術(shù)均為同一嫻熟的醫(yī)生進行。用SPSS11.0軟件對資料進行統(tǒng)計分析,P0.05表示差異有顯著意義。 結(jié)果: 1.調(diào)節(jié)功能:所有近視組患者LASIK術(shù)后第1周的單眼及雙眼調(diào)節(jié)幅度較術(shù)前下降(P0.05),然后逐漸恢復(fù),至術(shù)后1月時超過術(shù)前水平,且與正視組相比無顯著差異。 2.視疲勞癥狀:近視患者LASIK術(shù)后第1周視疲勞癥狀均較術(shù)前明顯加重,隨時間推移,視疲勞癥狀逐漸減輕,至術(shù)后1月視疲勞癥狀較術(shù)前明顯減輕。 3.相關(guān)性分析顯示:術(shù)后1周時調(diào)節(jié)幅度的降低程度與術(shù)前屈光度成正相關(guān);近視組視疲勞癥狀與雙眼調(diào)節(jié)幅度呈負(fù)相關(guān)。 結(jié)論: 1.LASIK術(shù)后患者的單眼及雙眼調(diào)節(jié)幅度均有短暫下降,術(shù)后第1周顯著下降,以后逐漸恢復(fù)。 2.LASIK術(shù)后視疲勞癥狀和調(diào)節(jié)幅度的變化有關(guān)。 3.LASIK手術(shù)對近視患者的眼調(diào)節(jié)幅度未產(chǎn)生不良影響。
[Abstract]:Objective: excimer laser corneal refractive surgery has been developed in China for nearly two decades. It has taken off heavy glasses for countless patients with ametropia, especially myopic patients. Today, when people pursue high efficiency work and high quality of life, it has also become a reliable choice for many people who pursue fashion and wear glasses. However, in clinical work, we found that some myopic patients had symptoms of visual fatigue of close working early after operation, which interfered with normal work and learning, which should attract our attention. The purpose of this study was to investigate the effect of excimer laser in situ grinding on the amplitude of monocular and binocular adjustment in myopic patients after operation, and to explore the relationship between the change of adjustment amplitude and visual fatigue symptoms, so as to provide a certain clinical basis for the prediction of visual quality after LASIK.
