遠(yuǎn)視兒童應(yīng)用1%環(huán)噴托酯散瞳驗光的研究

【摘要】:【目的】探討遠(yuǎn)視兒童應(yīng)用1%環(huán)噴托酯滴眼液以及聯(lián)合應(yīng)用表面麻醉藥丙美卡因滴眼液的遠(yuǎn)視屈光度和瞳孔散大值的峰值時間,并比較應(yīng)用1%環(huán)噴托酯與1%阿托品散瞳驗光的屈光度值的差異。 【方法】采用簡單隨機化分組方法將71例(141眼)遠(yuǎn)視兒童(年齡7.9±2.1歲)分為A組(Pro+Cyc組)和B組(NS+Cyc組),所有兒童均進行視力、眼位、眼球運動、裂隙燈及眼底檢查。散瞳驗光檢查步驟如下:(1)A組應(yīng)用鹽酸丙美卡因滴眼液滴眼1次;5分鐘后應(yīng)用1%環(huán)噴托酯滴眼3次,每次1滴,間隔10分鐘;(2)B組應(yīng)用生理鹽水滴眼1次,5分鐘后應(yīng)用1%環(huán)噴托酯滴眼,用法同A組。對A、B兩組分別于1%環(huán)噴托酯首次滴眼后30、40、50、60、70分鐘進行電腦驗光和瞳孔直徑測量。兩組均于1周后給予1%阿托品凝膠滴眼,每天3次,連用3天后重復(fù)上述測量,分別為A’組及B’組。采用SPSS16.0軟件包對數(shù)據(jù)進行統(tǒng)計學(xué)分析。 【結(jié)果】 (1)A組和B組屈光度的峰值分別出現(xiàn)在1%環(huán)噴托酯首次滴眼后50分鐘和60分鐘。 (2)A組遠(yuǎn)視屈光度的峰值(+4.44±2.34 D)小于A’組屈光度均值(+4.86±2.29D),兩者差異有顯著的統(tǒng)計學(xué)意義(t=11.16,P0.01);B組遠(yuǎn)視屈光度的峰值(+4.50±2.19 D)小于B’組屈光度均值(+5.04±2.10 D),兩者差異亦有顯著的統(tǒng)計學(xué)意義(t=11.44,P0.01)。A組遠(yuǎn)視屈光度的峰值與A’組屈光度值的差值(0.42±0.32 D)小于B組遠(yuǎn)視屈光度的峰值與B’組屈光度值的差值(0.54±0.39D),兩差值之間的差異有統(tǒng)計學(xué)意義(t=-1.99,P=-0.48)。 (3)A組和B組瞳孔直徑的峰值分別出現(xiàn)在1%環(huán)噴托酯首次滴眼后50分鐘和60分鐘,均與其屈光度的峰值時間一致。 (4)A’組與A組屈光度峰值的差值與A’組屈光度值、B’組與B組屈光度峰值的差值與B’組的屈光度值均無相關(guān)性;A’組與A組屈光度峰值的差值與年齡無相關(guān)性,B’組與B組屈光度峰值的差值與年齡呈負(fù)相關(guān)。 (5)A'-A50min組、B'-B60min組伴有內(nèi)斜視及不伴有內(nèi)斜視兩組之間的屈光度差值的均值之間分別進行兩兩比較,差異均無統(tǒng)計學(xué)意義(P0.05)。 [結(jié)論]在我國3-14歲遠(yuǎn)視兒童中,單獨應(yīng)用1%環(huán)噴托酯滴眼3次,在首次滴眼后50分鐘至70分鐘驗光可測得其最大的屈光度值;1%環(huán)噴托酯聯(lián)合應(yīng)用表面麻醉劑丙美卡因可使其峰值時間提前約10分鐘,并可增強1%環(huán)噴托酯的睫狀肌麻痹作用。單獨應(yīng)用1%環(huán)噴托酯或者丙美卡因與1%環(huán)噴托酯聯(lián)合應(yīng)用,均不能完全代替1%阿托品的睫狀肌麻痹作用。
[Abstract]:[Objective] To study the peak time of 1% cyclopentol eye drops and the peak time of the eye drops of the combined application of proparacaine eye drops, and to compare the difference of the refractive power of 1% cyclopentol and 1% atropine for the mydriasis. [Methods] 71 (141 eyes) hypermetropia children (aged 7.9 to 2.1 years) were divided into group A (Pro + Cyc group) and group B (NS + Cyc group) by a simple and randomized grouping method. All children were divided into group A (Pro + Cyc group) and group B (NS + Cyc group), and all children underwent visual, ocular, eye movement, slit lamp and fundus examination. Analysis. [Results] (1) The peak of the diopters of Group A and Group B respectively occurred in 50 minutes after the first drop of the first drop of the 1% ring spray. The difference between the peak value of the hyperopic power of group A and the power value of group A '(0.42-0.32D) was less than the difference between the peak value of the group B's hyperopic power and the value of the B' group (0.54, 0.39D), and the difference between the two differences was of statistical significance (t =-1.99, P =-0.48). (3) The peak of the pupil diameter in group A and group B respectively occurred in 50 minutes and 60 minutes after the first drop of the 1% ring-spray, both of which The peak time of the luminosity is consistent. (4) The difference between the peak value of the group A and the power of the group A and the power value of the group A ', the difference between the group B' and the group B's power peak value is not related to the power value of the B 'group, and the difference between the group A' and the group A power peak is not related to the age, and the B 'and the B group power peak (5) In group A '-A50min, B'-B60min group was associated with internal strabismus and the mean value of the power difference between the two groups without internal strabismus.
