急性區(qū)域性隱匿性外層視網(wǎng)膜病變頻域光學(xué)相干斷層掃描圖像的特征分析

【摘要】:目的觀察急性區(qū)域性隱匿性外層視網(wǎng)膜病變(acute zonal occult outer retinopathy,AZOOR)的頻域光學(xué)相干斷層掃描圖像(optical coherence tomography,OCT)的特征。方法臨床確診為AZOOR患者5例(5眼)納入研究,健眼為對(duì)照眼。所有患者均行最佳矯正視力檢查,并行OCT檢查患者的黃斑區(qū),觀察并分析患者的OCT圖像特征,并和健眼比較分析黃斑區(qū)的厚度差異和結(jié)構(gòu)改變。結(jié)果 OCT檢查提示患者存在不同程度的視網(wǎng)膜外層損害,損害的位置和范圍不同,其共同特征是外界膜層光帶信號(hào)強(qiáng)度不均勻,局部顯示模糊甚至局部缺損,局部區(qū)域向下凹陷。光感受器內(nèi)節(jié)/外節(jié)光帶出現(xiàn)強(qiáng)度減弱和部分區(qū)域斷裂、消失的現(xiàn)象。而在幾乎所有的患眼病變區(qū)域視錐細(xì)胞外節(jié)尖端層模糊或消l 失,視網(wǎng)膜色素上皮層則沒有顯著的結(jié)構(gòu)改變。患眼中心凹處外界膜-視網(wǎng)膜色素上皮層厚度為(83.20±9.90)μm,健眼為(101.80±2.23)μm,差異有統(tǒng)計(jì)學(xué)意義(t=-4.09,P0.05)。Pearson相關(guān)分析顯示患眼與健眼外界膜-視網(wǎng)膜色素上皮層厚度的差值,與患眼BCVA呈負(fù)相關(guān)(r=-0.94,P0.05)。結(jié)論 AZOOR患者的視網(wǎng)膜損害多位于外層,其中視錐細(xì)胞外節(jié)尖端層損害最為顯著;患者均有不同程度的外界膜層至視網(wǎng)膜色素上皮層變薄的現(xiàn)象,可能是導(dǎo)致患者視力下降的主要原因。
[Abstract]:Objective to observe the characteristics of frequency domain optical coherence tomography (optical coherence tomography,OCT) in acute regional occult outer retinal disease (acute zonal occult outer retinopathy,AZOOR). Methods five patients (5 eyes) with AZOOR were included in the study, and the healthy eyes were used as control eyes. The best corrected visual acuity was performed in all patients, and the macular area was examined by OCT. The OCT image features of the patients were observed and analyzed, and the thickness difference and structural changes of macular area were compared with those of healthy eyes. Results OCT showed that there were different degrees of retinal outer layer damage, and the location and scope of the damage were different. The common characteristics were uneven signal intensity of external film light band, local blurred or even local defect, and local depression. The intensity of the inner / outer segment of the photoreceptor is weakened and some areas are broken and disappeared. However, in almost all the affected areas, the tip of the cone was blurred or lost, while there was no significant structural change in the retinal pigment epithelial layer.
