【摘要】：目的探討鼻內(nèi)鏡下射頻治療下鼻道穹隆部出血的臨床療效。方法將2011年11月至2014年11月間收治的182例下鼻道穹隆部位出血患者分為治療組和對照組,治療組93例,對照組89例,治療組進行鼻內(nèi)鏡下射頻治療,對照組進行傳統(tǒng)凡士林紗條填塞治療,對比兩組臨床療效。結(jié)果治療組1次治愈90例,治愈率為96.77%;對照組1次治愈45例,1次治愈率為50.56%。治療組的止血效果顯著高于對照組,差異有顯著性(P0.05)。隨訪3個月后,治療組4例復(fù)發(fā),復(fù)發(fā)率為4.30%;對照組復(fù)發(fā)16例,復(fù)發(fā)率為16.85%。治療組復(fù)發(fā)率顯著低于對照組,差異有顯著性(P0.05)。治療組經(jīng)治療后發(fā)熱、頭痛發(fā)生率為2.15%,鼻痛發(fā)生率為7.53%,創(chuàng)面再出血發(fā)生率為9.68%,黏膜糜爛發(fā)生率為5.38%;對照組經(jīng)治療后發(fā)熱、頭痛發(fā)生率為4.49%,鼻痛發(fā)生率為17.98%,創(chuàng)面再出血發(fā)生率為29.21%,黏膜糜爛發(fā)生率為25.84%;對照組不良反應(yīng)發(fā)生率明顯高于治療組,差異有顯著性(P0.05)。結(jié)論鼻內(nèi)鏡下射頻治療下鼻道穹隆部出血治療時間短,具有并發(fā)癥少、復(fù)發(fā)率低、視野清晰、定位準(zhǔn)確、止血迅速、損傷小等優(yōu)點,其療效明顯優(yōu)于傳統(tǒng)止血療法,是臨床上治療下鼻道穹隆部出血理想的方法。
[Abstract]:Objective to investigate the clinical effect of radiofrequency therapy under nasal endoscope in the treatment of inferior nasal fornix hemorrhage. Methods 182 patients with hemorrhage in inferior nasal fornix from November 2011 to November 2014 were divided into treatment group (n = 93) and control group (n = 89). The treatment group was treated with endoscopic radiofrequency therapy. The control group was treated with traditional Vaseline gauze filling, and the clinical efficacy of the two groups was compared. Results 90 cases (96.77%) were cured in the treatment group and 50.56% in the control group. The hemostatic effect of the treatment group was significantly higher than that of the control group (P 0.05). After 3 months follow-up, the recurrence rate was 4.30% in the treatment group and 16.85% in the control group. The recurrence rate in the treatment group was significantly lower than that in the control group (P 0.05). In the treatment group, the incidence of fever was 2.15%, the incidence of nasal pain was 7.53%, the incidence of wound rebleeding was 9.68%, and the incidence of mucous erosion was 5.38%. In the control group, the incidence of fever was 4.49%, the incidence of nasal pain was 17.98%, the incidence of wound rebleeding was 29.21%, and the incidence of mucous erosion was 25.84%. The incidence of adverse reactions in the control group was significantly higher than that in the treatment group (P 0.05). Conclusion the treatment time of inferior nasal canal fornix hemorrhage under nasal endoscope is short, the recurrence rate is low, the visual field is clear, the location is accurate, the hemostasis is rapid and the injury is small, and its curative effect is obviously better than that of traditional hemostatic therapy. It is an ideal method for the treatment of hemorrhage in the fornix of inferior nasal canal.
