【摘要】：背景:體外擴(kuò)增的自體脂肪來源干細(xì)胞具有多向分化的潛能,在組織工程和再生醫(yī)學(xué)領(lǐng)域具有廣闊的應(yīng)用前景。目的:觀察自體脂肪來源干細(xì)胞移植受損人鼻黏膜組織的結(jié)構(gòu)和功能變化。方法:選取因物理或化學(xué)因素造成鼻黏膜損害的患者10例,其中6例黏膜瘢痕,4例黏膜破潰,抽取自體脂肪組織進(jìn)行脂肪來源干細(xì)胞體外分離、培養(yǎng)、擴(kuò)增,移植前進(jìn)行質(zhì)量安全性檢測。6例患者在黏膜瘢痕基底部、4例患者在黏膜破潰處注射移植脂肪來源干細(xì)胞(1×107/cm2),共注射3次,每2次間隔15 d。末次注射移植后30,90,150 d分別行鼻腔容積、最小橫斷面積、黏膜纖毛清除功能測定,其中3例患者在移植前和移植后30 d,取0.1 cm×0.1 cm黏膜組織,進(jìn)行蘇木精-伊紅染色、Masson三色染色、AB-PAS染色。結(jié)果與結(jié)論:脂肪來源干細(xì)胞注射移植后,所有患者臨床癥狀得到緩解。與移植前比較,脂肪來源干細(xì)胞注射移植后30,90,150 d鼻腔容積及鼻腔最小橫斷面積有所減少(P0.05),鼻黏膜纖毛清除功能有所改善,但差異無顯著性意義(P0.05)。與移植前相比,脂肪來源干細(xì)胞移植30 d時3例患者鼻黏膜炎癥明顯減輕,膠原纖維排列整齊、沉積減少,杯狀細(xì)胞分泌的黏蛋白增多。結(jié)果證明了體外擴(kuò)增的脂肪來源干細(xì)胞可以重建鼻黏膜結(jié)構(gòu)和恢復(fù)其功能。
[Abstract]:Background: autologous adipose-derived stem cells (ADSCs) expanded in vitro have the potential for multidirectional differentiation and have broad application prospects in tissue engineering and regenerative medicine. Objective: to observe the structural and functional changes of human nasal mucosa after autologous adipose-derived stem cell transplantation. Methods: 10 patients with nasal mucosal damage caused by physical or chemical factors were selected, including 6 cases of mucosal scar and 4 cases of mucosal rupture. Autologous adipose tissue was extracted and isolated, cultured, and amplified from adipose-derived stem cells in vitro. Before transplantation, 6 patients were injected with transplanted adipose-derived stem cells (1 脳 10 ~ 7 / cm2) at the base of cicatricial base of mucosa, and 4 patients were injected with transplanted adipose-derived stem cells (1 脳 10 ~ 7 / cm2) at the base of mucosal cicatricial base, for 15 d every 2 times. Nasal cavity volume, minimum cross-sectional area and mucociliary clearance function were measured 30 and 90150 days after the last injection. The mucosal tissue of 0.1 cm 脳 0.1 cm was stained with hematoxylin-eosin in 3 patients before transplantation and 30 days after transplantation. Masson trichrome staining, AB-PAS staining. Results & conclusion: after transplantation of adipose-derived stem cells, the clinical symptoms of all patients were relieved. Compared with pre-transplantation, the volume of nasal cavity and the minimum cross-sectional area of nasal cavity were decreased at 30 and 90150 days after adipose-derived stem cell transplantation (P0.05), and the ciliary clearance function of nasal mucosa was improved, but there was no significant difference (P0.05). At 30 days after adipose-derived stem cell transplantation, the inflammation of nasal mucosa in 3 patients was significantly alleviated, collagen fibers were arranged neatly, deposition decreased, and mucin secreted by goblet cells increased. The results showed that the adipose-derived stem cells expanded in vitro could reconstruct the nasal mucosa structure and restore its function.
