【摘要】：目的探討超聲乳化白內(nèi)障吸出聯(lián)合人工晶狀體植入術治療原發(fā)性房角關閉(primary angle-closure,PAC)或原發(fā)性閉角型青光眼(primary angle-closure glaucoma,PACG)合并白內(nèi)障的療效。方法回顧性分析2011年3月至2012年12月于我院住院治療的PAC或PACG合并白內(nèi)障患者60例(60眼)的臨床資料,分為PAC組(28例)和PACG組(32例),所有患者均采用超聲乳化人工晶狀體植入術,術前、術后1 d、1個月、6個月采用國際標準視力表檢查患者最佳矯正視力,采用光學相干斷層掃描觀察患者前房角度(anterior chamber angle,ACA),并采用非接觸式眼壓計測量患者眼壓,同時觀察術后并發(fā)癥發(fā)生情況。結果 PAC組和PACG組術前最佳矯正視力分別為0.31±0.21、0.32±0.19,術后6個月分別為0.57±0.28、0.58±0.26,與術前比較,兩組患者術后最佳矯正視力均有較大程度的改善,差異均有統(tǒng)計學意義(均為P0.05);兩組術后組內(nèi)及組間不同時間點間最佳矯正視力相比,差異均無統(tǒng)計學意義(均為P0.05)。與術前比較,兩組術后不同時間眼壓均顯著降低,差異均有統(tǒng)計學意義(均為P0.05);兩組術后組內(nèi)不同時間點間眼壓比較,差異均無統(tǒng)計學意義(均為P0.05);PAC組術后不同時間點眼壓均低于PACG組,但差異均無統(tǒng)計學意義(均為P0.05)。與術前相比,兩組術后ACA均增寬,差異均有統(tǒng)計學意義(均為P0.05);兩組術后組內(nèi)及組間不同時間點間ACA相比,差異均無統(tǒng)計學意義(均為P0.05)。結論超聲乳化白內(nèi)障吸出聯(lián)合人工晶狀體植入術治療PAC或PACG合并白內(nèi)障患者短期內(nèi)療效顯著,但遠期療效仍有待進一步觀察。
[Abstract]:Objective to evaluate the efficacy of phacoemulsification and intraocular lens implantation in the treatment of primary angle closure (primary angle-closure,PAC) or primary angle closure glaucoma (primary angle-closure glaucoma,PACG) with cataract. Methods the clinical data of 60 patients (60 eyes) with PAC or PACG complicated with cataract who were hospitalized in our hospital from March 2011 to December 2012 were retrospectively analyzed. They were divided into two groups: PAC group (28 cases) and PACG group (32 cases). All patients were treated with phacoemulsification and IOL implantation. The best corrected visual acuity was measured by international standard visual acuity scale 1 day, 1 month and 6 months after operation. The anterior chamber angle (anterior chamber angle,ACA) of the patients was observed by optical coherence tomography (Oct), and the intraocular pressure (IOP) was measured by non-contact tonometer. The postoperative complications were observed at the same time. Results the best corrected visual acuity was 0.31 鹵0.21, 0.32 鹵0.19 in PAC group and 0.32 鹵0.19 in PACG group, and 0.57 鹵0.28 and 0.58 鹵0.26 in 6 months after operation, respectively. The difference was statistically significant (all P0.05); There was no significant difference in the best corrected visual acuity between the two groups at different time points (P 0.05). Compared with pre-operation, IOP decreased significantly in two groups at different time after operation (all P 0.05); there was no significant difference in IOP between two groups at different time points after operation (all P 0.05). IOP in PAC group was lower than that in PACG group at different time points after operation, but there was no significant difference (P 0.05). Compared with pre-operation, the postoperative ACA of the two groups increased significantly (P 0.05), and there was no significant difference in ACA between the two groups at different time points (P 0.05). Conclusion Phacoemulsification cataract extraction combined with intraocular lens implantation is effective in the treatment of PAC or PACG with cataract in a short time, but the long-term effect remains to be further observed.
