玻璃體積血的病因分析

【摘要】：目的:分析我院126例玻璃體積血患者的病因,探討玻璃體積血病因與性別及年齡的相關(guān)性,為選擇合理的治療方法提供依據(jù)。 方法:對我院2005年6月~2010年3月期間以玻璃體積血為首診的126例(130只眼)住院患者的病例資料進(jìn)行回顧性分析。 結(jié)果:在所有玻璃體積血的患者中,增殖型糖尿病視網(wǎng)膜病變(PDR)引起者最多54例(42.86%),眼外傷次之37例(29.37%),視網(wǎng)膜靜脈周圍炎和靜脈阻塞(RVO)引起者分別為8例(6.35%)、15例(11.90%)。76例男性患者中眼外傷引起玻璃體積血者占首位30例(39.47%),其次為PDR 27例(35.53%),再次為RVO 7例(9.21%),視網(wǎng)膜靜脈周圍炎6例8只眼(7.89%)。50例女性患者中PDR 27例(54.00%),RVO 8例(16.00%),眼外傷7例(14.00%),視網(wǎng)膜靜脈周圍炎2例(4.00%)。男女病因構(gòu)成存在統(tǒng)計學(xué)差異(χ2=18.21,p0.05)。44歲以下的青年患者中,眼外傷及視網(wǎng)膜靜脈周圍炎患者占多數(shù),分別為25例(55.56%)、8例10只眼(17.78%),PDR 6例(13.33%)。45歲~59歲中年患者中,PDR 21例22只眼(56.76%),眼外傷者7例(18.92%),RVO 6例(16.22%),三者為該組患者玻璃體積血的主要原因。60歲以上的老年患者中,PDR 27例28只眼(61.36%),眼外傷者5例(11.36%),RVO 7例(15.91%)。各年齡組病因構(gòu)成存在統(tǒng)計學(xué)差異(χ2=81.52,p0.005)。 結(jié)論:PDR、眼外傷、RVO、視網(wǎng)膜靜脈周圍炎是引起玻璃體積血最常見的原因;眼外傷、PDR、RVO、視網(wǎng)膜靜脈周圍炎是男性患者玻璃體積血的主要原因,PDR、RVO、眼外傷是女性患者玻璃體積血的主要原因;眼外傷、視網(wǎng)膜靜脈周圍炎是44歲以下青年患者玻璃體積血的主要原因。PDR、眼外傷、RVO是45~59歲中年患者玻璃體積血的主要病因。PDR、RVO是60歲以上老年患者玻璃體積血的主要原因。
[Abstract]:Aim: to analyze the etiology of vitreous hemorrhage in 126 patients with vitreous hemorrhage in our hospital, and to explore the relationship between the cause of vitreous hemorrhage and sex and age, so as to provide the basis for selecting a reasonable treatment method. Methods: the clinical data of 126 patients (130 eyes) who were first diagnosed with vitreous blood from June 2005 to March 2010 in our hospital were retrospectively analyzed. Results: of all the patients with vitreous hemorrhage, 54 (42.86%) were (PDR)-induced proliferative diabetic retinopathy, 37 (29.37%) were extraocular injury, and 54 (42.86%) were proliferative diabetic retinopathy, and 37 (29.37%) were extraocular injury. Among the 76 male patients, 30 (39.47%) had vitreous hemorrhage caused by ocular trauma, and 8 (6.35%) and 15 (11.90%) were caused by (RVO) in peripheral retinal vein and vein occlusion, respectively. The second was PDR in 27 cases (35.53%), RVO in 7 cases (9.21%), Periretinal vein inflammation in 8 eyes (7.89%) in 6 cases, PDR in 27 cases (54.00%), RVO in 8 cases, 16.00%) in 50 women. There were 7 cases (14.00%) of ocular trauma and 2 cases (4.00%) of periretinal vein inflammation. Among the young patients under 44 years of age, 25 cases (55.56%) and 8 cases (10 eyes) (17.78%) with ocular trauma and peripheral retinal vein inflammation were found in most of the young patients under the age of 44 years old (蠂 2 = 18.21, p 0.05), and the difference was statistically significant between men and women (蠂 2 / 18.21, p 0.05). Among the middle-aged patients aged 45 ~ 59 years, 22 eyes (56.76%) were PDR, 7 eyes (18.92%), RVO) were extraocular injury, 6 (16.22%) were), RVO, 7 cases (18.92%) were PDR, 22 eyes (56.76%) were PDR, and 6 cases (16.22%) were extraocular injury. Among the elderly patients over 60 years old, 28 eyes (61.36%) of PDR patients and 5 (11.36%) cases (15.91%) of extraocular injuries were diagnosed as vitreous hemorrhage, including 27 cases (61.36%) of PDR and 5 cases (11.36%) (15.91%) of extraocular injury. There were statistical differences among the age groups (蠂 2 = 81.52, p 0.005). Conclusion: the most common cause of vitreous hemorrhage is PDR, 's eye injury and RVO, 's periretinal vein periarthritis. Ocular trauma, PDR,RVO, retinal vein periarthritis is the main cause of vitreous hemorrhage in male patients, and PDR,RVO, ocular trauma is the main cause of vitreous hemorrhage in female patients. Ocular trauma, periretinal vein inflammation is the main cause of vitreous hemorrhage in young patients under 44 years old. PDR, ocular trauma, and RVO are the main causes of vitreous hemorrhage in 45-59-year-old middle-aged patients. RVO is the main cause of vitreous hemorrhage in elderly patients over 60 years old.
