【摘要】：目的探討內(nèi)鏡下低溫等離子手術(shù)(耳內(nèi)鏡下鼓膜打孔術(shù)+鼻內(nèi)鏡下腺樣體和/或下鼻甲消融術(shù)/功能性鼻內(nèi)鏡術(shù))治療"分泌性中耳炎"的療效,并發(fā)癥,優(yōu)勢及復(fù)發(fā)率。資料與方法分析研究156例病史超過3個(gè)月,由腺樣體肥大和慢性鼻-鼻竇炎引起的分泌性中耳炎患者,所有病例針對病因先行鼻內(nèi)鏡下等離子腺樣體和/或下鼻甲消融術(shù)/功能性鼻內(nèi)鏡術(shù),研究組(78例)患者同期行等離子鼓膜打孔術(shù),對照組(78例)行鼓膜置管術(shù)。結(jié)果所有患者隨訪6個(gè)月,以術(shù)后1周、1月、3月及6月時(shí)的檢查結(jié)果作對比,研究組78例患者鼓膜打孔處約1-3月愈合,兩組治愈好轉(zhuǎn)率經(jīng)比較,P值均0.001,差異有統(tǒng)計(jì)學(xué)意義。并發(fā)癥以中耳感染、流膿為主要參考指標(biāo),對照組的并發(fā)癥在不同時(shí)段均明顯高于研究組,經(jīng)卡方檢驗(yàn),術(shù)后1周P值為0.043,其余均P0.001,兩組患者在并發(fā)癥的發(fā)生率上有顯著差別,差異有統(tǒng)計(jì)學(xué)意義。結(jié)論內(nèi)鏡下低溫等離子手術(shù)(腺樣體和/或下鼻甲消融術(shù)/功能性鼻內(nèi)鏡術(shù)+鼓膜打孔術(shù))治療分泌性中耳炎具有療效好,創(chuàng)傷小,并發(fā)癥少,復(fù)發(fā)率低等優(yōu)勢。
[Abstract]:Objective to investigate the efficacy, complications, advantages and recurrence rate of endoscopic hypothermic plasma surgery (endoscopic tympanoplasty and / or inferior turbinate ablation / functional endoscopy) in the treatment of secretory otitis media. Materials and methods 156 patients with secretory otitis media caused by adenoid hypertrophy and chronic rhinosinusitis were analyzed. All patients were treated with plasma adenoid and / or inferior turbinate ablation / functional rhinoscopy under nasal endoscopy. Plasma tympanoplasty was performed in 78 patients in the study group and tympanoplasty was performed in 78 patients in the control group at the same time. The patients in the study group (n = 78) were treated with plasma tympanoplasty and the control group (n = 78). Results all the patients were followed up for 6 months. Compared with the results at 1 week, 1 month, 3 months and 6 months after operation, 78 patients in the study group healed around 1-3 months in the tympanic perforations. The recovery rates of the two groups were compared, P = 0.001, P < 0.01, respectively. The difference is statistically significant. The complications in the control group were significantly higher than those in the study group at different time points. After chi-square test, the P value was 0.043 at one week after operation, and the rest were all P0.001, and the other complications were significantly higher in the control group than in the study group at different time points. There was a significant difference in the incidence of complications between the two groups, the difference was statistically significant. Conclusion Endoscopic hypothermic plasma surgery (adenoid and / or inferior turbinate ablation / functional endoscope tympanoplasty) has the advantages of good curative effect, less trauma, less complications and lower recurrence rate in the treatment of secretory otitis media.
