【摘要】：目的:探討預(yù)測聲門暴露困難的可靠指標(biāo),分析術(shù)后咽喉部疼痛與術(shù)中支撐喉鏡使用時間及喉部和舌根所受壓強(qiáng)的關(guān)系。方法:對50例實(shí)行支撐喉鏡手術(shù)的患者術(shù)前測量和聲門暴露可能相關(guān)的指標(biāo)并進(jìn)行統(tǒng)計(jì)學(xué)分析,包括:性別、年齡、體質(zhì)指數(shù)、改良Mallampati分級、張口度、頸圍、胸頦間距、甲頦間距、舌頦間距、水平甲頦間距、垂直甲頦間距、頸后仰度、甲頦夾角,術(shù)中記錄聲門暴露的Cormack-Lehane評分、聲門暴露分級、暴露聲門所需時間、手術(shù)持續(xù)時間、作用喉和舌根部的壓力,術(shù)后記錄咽喉部疼痛情況,并分析疼痛的相關(guān)因素。結(jié)果:體質(zhì)指數(shù)≥25和全伸展體位下的甲頦間距6cm(均P0.05)是預(yù)測聲門暴露困難的可靠指標(biāo);聲門暴露所需時間[(1.96±1.28)min]及舌根和喉部所受壓強(qiáng)[(292±109)mmHg]在聲門暴露困難患者中顯著增加,22例(44%)患者術(shù)后主訴喉痛,其程度和手術(shù)時間[(39±31)min]有關(guān)(P0.05),而和壓強(qiáng)無關(guān)(P0.05)。結(jié)論:體質(zhì)指數(shù)和全伸展體位下甲頦間距可作為預(yù)測聲門暴露困難的可靠指標(biāo);術(shù)中喉部和舌根所受壓強(qiáng)是術(shù)后咽喉部疼痛的一個重要因素,統(tǒng)計(jì)學(xué)表明僅手術(shù)時間和術(shù)后疼痛有顯著相關(guān)性。
[Abstract]:Objective: to explore the reliable index of predicting glottic exposure difficulties and to analyze the relationship between postoperative larynx pain and the duration of operation of laryngoscope and the pressure on larynx and tongue root. Methods: preoperative measurements and glottic exposure were analyzed in 50 patients undergoing laryngoscopic surgery, including sex, age, body mass index, modified Mallampati grade, mouth opening, neck circumference, and mental distance. Cormack-Lehane score of glottis exposure, glottis exposure grade, duration of glottis exposure, duration of operation were recorded.
