不同飲食方式在減少保留懸雍垂腭咽成形術(shù)后切口開裂的臨床研究

【摘要】: 目的 探討一種適合的飲食方式減少保留懸雍垂腭咽成形術(shù)(H-UPPP)后切口的開裂。 方法 收集了2008年12月~2010年1月間于我科行保留懸雍垂腭咽成形術(shù)(H-UPPP)患者共136例,按患者意愿隨機(jī)分成三組,A組42人,術(shù)中下鼻飼,禁止經(jīng)口進(jìn)食,給予鼻飼高營養(yǎng)食物5-7天。B組50例,術(shù)后第2天進(jìn)冷流質(zhì),第3-6d進(jìn)半流食,第7d改為軟食。C組44例,術(shù)后禁止進(jìn)食,經(jīng)靜脈充分補(bǔ)給能量和液體,第5-7d經(jīng)口進(jìn)流食。術(shù)后一周內(nèi)對三組術(shù)后患者縫合切口開裂及繼發(fā)性出血情況進(jìn)行比較。 結(jié)果 A組和C組病人術(shù)后縫合切口開裂及繼發(fā)性出血較B組減少,差異有統(tǒng)計學(xué)意義P0.05)。A組和C組間無統(tǒng)計學(xué)差異。 結(jié)論 H-UPPP患者術(shù)后4天內(nèi)不主張經(jīng)口進(jìn)食,采用鼻飼飲食或靜脈營養(yǎng)可以減少術(shù)后患者的切口開裂及繼發(fā)性出血。
[Abstract]:Objective to explore a suitable diet to reduce incision cracking after uvulopalatopharyngoplasty (H-UPPP). Methods A total of 136 patients undergoing uvulopalatopharyngoplasty (H-UPPP) from December 2008 to January 2010 were randomly divided into three groups according to the wishes of the patients. 50 patients in group B received cold fluid on the second day, half fluid at the 3rd to 6th day, and then soft food on the 7th day. At the 5th-7th day, the food was fed through the mouth. A week after operation, the suture incision cracking and secondary bleeding were compared among the three groups. Results the suture incision cracking and secondary bleeding in group A and group C were less than those in group B (P0.05). There was no significant difference between group C and group A (P0.05). Conclusion H-UPPP patients do not advocate oral feeding within 4 days, nasal feeding diet or intravenous nutrition can reduce incision cracking and secondary bleeding.
