國產(chǎn)親水性丙烯酸人工晶狀體的生物相容性研究

【摘要】: 目的: 軟性人工晶狀體(Intraocular Lens IOL)是目前白內(nèi)障術(shù)中使用的主流人工晶狀體,而我國軟性IOL的研發(fā)尚處于起步階段。本研究擬對天津泰達(dá)生物科技有限公司研發(fā)的軟性親水性丙烯酸IOL進(jìn)行體外的細(xì)胞毒性檢測,并將其植入兔眼內(nèi)以綜合評價其生物相容性。 方法: 分為體外和體內(nèi)實(shí)驗(yàn)兩部分。 1.體外,選擇親水性丙烯酸、疏水性丙烯酸和硅凝膠三種軟性材料的IOL進(jìn)行生物相容性和細(xì)胞毒性的觀察和比較。分別提取人外周血的血小板、中性粒細(xì)胞,小鼠腹腔巨噬細(xì)胞、人眼結(jié)膜囊成纖維細(xì)胞置于三種IOL表面并培養(yǎng),觀察其細(xì)胞粘附生長的數(shù)量與狀態(tài)。 2.選擇表皮葡萄球菌標(biāo)準(zhǔn)菌株對以上三種軟性IOL及聚甲基丙烯酸甲酯(polymethyl methacrylate PMMA)IOL、肝素修飾PMMA IOL進(jìn)行細(xì)菌粘附的比較。 3.體內(nèi),依照國家標(biāo)準(zhǔn)和國家醫(yī)藥局發(fā)布的《眼內(nèi)IOL》標(biāo)準(zhǔn),選擇已臨床應(yīng)用的進(jìn)口親水性丙烯酸IOL作為對照IOL,進(jìn)行家兔眼內(nèi)植入的初步研究。選擇日本大耳白兔26只,分為4組:第一組為正常對照,2只兔,第二組16只兔,行雙眼超聲乳化晶狀體吸除后,一眼植入實(shí)驗(yàn)IOL,另一眼植入對照IOL;第三組4只兔,晶狀體乳化吸除后一眼植入實(shí)驗(yàn)IOL,一眼不植IOL;第四組4只眼,晶狀體乳化吸除后一眼植入對照IOL,另一眼不植入IOL,共計(jì)各有20枚實(shí)驗(yàn)和對照IOL植入兔眼內(nèi)。 4.分別于術(shù)后1周、1月、3月抽取兔眼前房水進(jìn)行白細(xì)胞計(jì)數(shù),并行裂隙燈檢察,觀測眼前節(jié)的反應(yīng)。于術(shù)后1月、3月分別處死第二組的5只兔,取出IOL進(jìn)行掃描電鏡檢查,并在同時期各處死第三、四組的2只兔,摘除眼球,行兔眼前、后節(jié)的病理觀察。 結(jié)果: 1.體外,血小板和中性粒細(xì)胞在親水性丙烯酸IOL表面粘附的最少,明顯少于疏水性丙烯酸和硅凝膠IOL;小鼠腹腔巨噬細(xì)胞在親水性丙烯酸IOL表面的粘附少于疏水性丙烯酸IOL,而與硅凝膠IOL無顯著性差異;人結(jié)膜囊成纖維細(xì)胞在疏水性丙烯酸IOL表面生長最密集,親水性丙烯酸IOL表面的成纖維細(xì)胞粘附的較少,但形態(tài)健康。 2.表皮葡萄球菌在親水性丙烯酸IOL上粘附的最少,與肝素修飾的IOL相當(dāng),在硅凝膠IOL表面粘附的最多。 3.兔眼內(nèi),實(shí)驗(yàn)國產(chǎn)親水性丙烯酸IOL植入術(shù)后7天,房水白細(xì)胞計(jì)數(shù)較對照組多,其余各觀察時間(30天、90天)的房水白細(xì)胞計(jì)數(shù)兩組間無顯著性差異。國產(chǎn)親水性丙烯酸IOL植入后角膜水腫、前房滲出及IOL表面附著物情況的裂隙燈檢查,與進(jìn)口親水性丙烯酸IOL相比均未見明顯區(qū)別。 4.IOL取出后掃描電鏡顯示,術(shù)后1個月,國產(chǎn)親水性丙烯酸IOL表面可見少量蛋白膜,巨噬細(xì)胞成團(tuán)分布,術(shù)后3個月,表面蛋白膜縮小,巨噬細(xì)胞單個分布,與進(jìn)口對照組之間無顯著性區(qū)別。 5.兔眼摘除后,兩組眼前節(jié)組織病理學(xué)檢查均表現(xiàn)為:術(shù)后1個月,前房內(nèi)少量滲出物,房角開放,術(shù)后3個月,滲出吸收,房角仍大部分開放,視網(wǎng)膜組織各層結(jié)構(gòu)大致正常。 結(jié)論: 1.國產(chǎn)親水性丙烯酸IOL在體外表現(xiàn)為較低的細(xì)胞粘附和細(xì)菌粘附,無細(xì)胞毒性。 2.兔眼內(nèi),國產(chǎn)親水性丙烯酸IOL與進(jìn)口對照IOL相比,同樣具有良好的葡萄膜相容性和較好的囊膜相容性。
