【摘要】：背景：鼻腔鼻竇惡性黑色素瘤是一種少見的惡性腫瘤,其預(yù)后差,目前臨床上常用的治療方案未能達(dá)到有效的控制。目的：通過與單純手術(shù)治療的對比,對術(shù)后放療在鼻腔鼻竇惡性黑色素瘤中的臨床應(yīng)用價值進(jìn)行系統(tǒng)評價。方法：檢索PubMed、Web of science、Ovid3個數(shù)據(jù)庫截止至2013年12月的文獻(xiàn),從中進(jìn)行篩選,并對納入結(jié)果進(jìn)行Meta分析。結(jié)局評價指標(biāo)為術(shù)后總體生存率、無瘤生存率和腫瘤局部控制率。結(jié)果：共納入19篇研究,涉及654例病例,其中術(shù)后放療組313例,單純手術(shù)組341例。術(shù)后放療組患者的1年、3年、5年總體生存率較單純手術(shù)組無顯著提高,其RR值和95%CI分別為1.03(0.92,1.14),1.05(0.88,1.27)和1.29(0.96,1.72)。術(shù)后無瘤生存率和腫瘤局部控制率也無顯著優(yōu)勢,1年、3年、5年無瘤生存率的RR值和95%CI分別為0.77(0.50,1.18),1.17(0.66,2.07)和0.80(0.46,1.39)；1年、3年、5年局部控制率的RR值和95%CI為1.07(0.87,1.32),1.11(0.80,1.56)和0.86(0.52,1.43；P=0.5)。結(jié)論：與單純手術(shù)治療相比,術(shù)后放療對鼻腔鼻竇惡性黑色素瘤患者的總體生存率、無瘤生存率和腫瘤局部控制率均無顯著改善。
[Abstract]:Background: malignant melanoma of nasal cavity and paranasal sinus is a rare malignant tumor with poor prognosis. Objective: to evaluate the clinical value of postoperative radiotherapy in malignant melanoma of nasal cavity and paranasal sinus. Methods: the literatures of PubMed,Web of science,Ovid3 database as of December 2013 were searched and screened, and the included results were analyzed by Meta. The outcome criteria were overall survival rate, tumor-free survival rate and tumor local control rate. Results: a total of 19 studies involved 654 cases, including 313 cases in postoperative radiotherapy group and 341 cases in simple operation group. The 1 year, 3 year and 5 year overall survival rate of postoperative radiotherapy group was not significantly higher than that of simple operation group. The RR value and 95%CI were 1. 03 (0. 92 鹵1. 14), 1. 05 (0. 88 鹵1. 27) and 1. 29 (0. 966 / 1. 72), respectively. There was no significant difference in tumor free survival rate and tumor local control rate. The RR and 95%CI of 1 year, 3 year and 5 year free survival rates were 0. 77 (0. 50 鹵1. 18), respectively. 1.17 (0.66U 2.07) and 0.80 (0.46L 1.39); The RR value and 95%CI of local control rate of 1 year, 3 years and 5 years were 1. 07 (0. 87 ~ 1. 32), 1. 11 (0. 80 ~ 1. 56) and 0. 86 (0. 52 ~ 1. 43 P0. 5) respectively. Conclusion: the overall survival rate, tumor-free survival rate and tumor local control rate of patients with malignant melanoma of nasal cavity and paranasal sinus treated by postoperative radiotherapy were not significantly improved compared with surgical treatment alone.
