遠(yuǎn)視兒童應(yīng)用1%環(huán)戊通與1%阿托品散瞳驗光的對照研究

【摘要】: 【目的】比較遠(yuǎn)視兒童先后應(yīng)用1%環(huán)戊通及1%阿托品散瞳電腦驗光的屈光度值,評估1%環(huán)戊通對我國遠(yuǎn)視兒童睫狀肌麻痹后驗光的效果。 【方法】所有兒童均進(jìn)行視力、眼位、眼球運動、裂隙燈及眼底檢查。散瞳驗光檢查步驟如下:(1)1%環(huán)戊通點眼2次散瞳電腦驗光(Cyc2-AR):應(yīng)用1%環(huán)戊通點眼2次,每次1滴,中間間隔10分鐘。第2次點眼后25分鐘進(jìn)行電腦驗光。(2)1%環(huán)戊通點眼3次散瞳電腦驗光(Cyc3-AR):行第一次電腦驗光后即刻再應(yīng)用1%環(huán)戊通點眼1次,15分鐘后進(jìn)行第二次電腦驗光。(3)1%阿托品散瞳電腦驗光(Atr-AR):對一眼遠(yuǎn)視屈光度≥+0.50D者,1周后應(yīng)用1%阿托品眼膏點眼,每日3次,共點3天后進(jìn)行第三次電腦驗光。 共收集完成以上三次散瞳電腦驗光的遠(yuǎn)視兒童105例(210眼),除外因重度弱視不能固視而影響電腦驗光檢測結(jié)果者6只眼,對其它204只眼進(jìn)行統(tǒng)計分析。 【結(jié)果】 (1)應(yīng)用Cyc2-AR、Cyc3-AR、Atr-AR三種散瞳方法驗光的屈光度值分別為+3.42±2.20D.+3.61±2.30D和+4.54±2.25D,三種散瞳方法驗光的屈光度值進(jìn)行兩兩比較,差異均有顯著的統(tǒng)計學(xué)意義(P0.001)。 (2)≤5歲與5歲兩組應(yīng)用三種散瞳方法驗光的屈光度值之間兩兩比較,其差異均有顯著的統(tǒng)計學(xué)意義;≤5歲組每兩種散瞳方法驗光的屈光度差值均大于5歲組的屈光度差值(P0.001)。 (3)在≤5歲和5歲兒童中,伴有內(nèi)斜視組每兩種散瞳方法驗光的屈光度差值均大于不伴有內(nèi)斜視組每兩種散瞳方法驗光的屈光度差值(P0.001)。 (4)在5歲不伴有內(nèi)斜視兒童中,Cyc2-AR、Cyc3-AR與Atr-AR的屈光度差值≤0.50D的眼數(shù)的百分率分別為70.4%和88.9%,差異有顯著的統(tǒng)計學(xué)意義(P=0.003)。在5歲伴有內(nèi)斜視的兒童中, Cyc2-AR、Cyc3-AR與Atr-AR的屈光度差值≤0.50D的眼數(shù)的百分率分別為41.6%和57.1%,差異無統(tǒng)計學(xué)意義(P=0.053)。在≤5歲兒童中, Cyc2-AR、Cyc3-AR與Atr-AR的屈光度差值≤0.50D的眼數(shù)的百分率分別為19.6%和21.7%,差異無統(tǒng)計學(xué)意義(P=0.797)。 (5)Cyc2-AR、Cyc3-AR、Atr-AR三種散瞳方法驗光的屈光度差值之間兩兩比較,與Atr-AR的屈光度值成正相關(guān),與遠(yuǎn)視兒童的年齡成負(fù)相關(guān)。 [結(jié)論](1)在中國3-12歲遠(yuǎn)視兒童中,應(yīng)用三種散瞳方法驗光的屈光度值從大到小依次為:Atr-AR、Cyc3-AR、Cyc2-AR;(2)≤5歲遠(yuǎn)視兒童應(yīng)選擇1%阿托品散瞳驗光,5歲不伴有內(nèi)斜視兒童可選擇1%環(huán)戊通點眼3次散瞳驗光,5歲伴有內(nèi)斜視兒童應(yīng)選擇1%阿托品進(jìn)行驗光;(3)遠(yuǎn)視兒童的屈光度愈高,1%環(huán)戊通與1%阿托品散瞳驗光的屈光度差值愈大;遠(yuǎn)視兒童的年齡愈大,1%環(huán)戊通與1%阿托品散瞳驗光的屈光度差值愈小
