【摘要】：目的探討慢性扁桃體炎患者是否有存在尿微量白蛋白含量異常，并發(fā)早期腎臟損害；分析尿微量白蛋白作為判斷慢性扁桃體炎早期腎臟損害的診斷指標(biāo)是否可靠；扁桃體切除術(shù)是否能夠降低尿微量白蛋白含量，成為防止慢性扁桃體炎并發(fā)早期腎臟損害的有效方法之一。 方法選取2012年3月～2013年2月青海大學(xué)附屬醫(yī)院耳鼻喉科慢性扁桃體炎患者84例，男性33例，女性51例，，年齡7～55歲。按病程分為3組,甲組：病程12月以上；乙組:病程36月以上；丙組；病程60月以上；在免疫檢驗(yàn)室收集12例非慢性扁桃體炎標(biāo)本正常者為對(duì)照組，男性7例，女性5例，年齡7～55歲。采用速率散射比濁法測(cè)定手術(shù)治療前、后尿微量白蛋白含量。對(duì)所采集數(shù)據(jù)進(jìn)行統(tǒng)計(jì)學(xué)分析。 結(jié)果1.甲組術(shù)前尿微量白蛋白含量與對(duì)照組尿微量白蛋白含量比較，p0.05，無(wú)統(tǒng)計(jì)學(xué)意義。其余各組術(shù)前尿微量白蛋白含量?jī)蓛啥嘀乇容^均為P＜0.05，差異有統(tǒng)計(jì)學(xué)意義。 2.甲組、乙組、丙組手術(shù)前后尿微量白蛋白含量比較均為P＜0.05，差異有統(tǒng)計(jì)學(xué)意義。 3.慢性扁桃體炎患者尿微量白蛋白含量為非正態(tài)性分布，慢性扁桃體炎患者病程與尿微量白蛋白含量秩相關(guān)分析rs=0.659，P0.001,說(shuō)明慢性扁桃體炎患者病程與尿微量白蛋白含量有相關(guān)性。結(jié)論1、病程60月以上的慢性扁桃體炎患者尿微量白蛋白排泄量超出正常范圍,可并發(fā)早期腎臟損害； 2、尿微量白蛋白作為判斷慢性扁桃體炎早期腎臟損害的診斷指標(biāo)有一定臨床指導(dǎo)價(jià)值，且隨著病程進(jìn)展其敏感性逐漸增加； 3、慢性扁桃體炎患者在扁桃體切除術(shù)術(shù)后尿微量白蛋白含量有明顯下降，扁桃體切除術(shù)對(duì)防治早期腎臟損害有明顯的療效。
[Abstract]:Objective to investigate the presence of urinary microalbumin in patients with chronic tonsillitis complicated with early renal damage, and to analyze the reliability of urinary microalbumin as a diagnostic index for early renal damage in chronic tonsillitis. Tonsillectomy is one of the effective methods to prevent early renal damage in chronic tonsillitis. Methods from March 2012 to February 2013, 84 patients with chronic tonsillitis in the department of otolaryngology, affiliated hospital of Qinghai University, including 33 males and 51 females, aged 755 years, were selected. According to the course of disease, group A: course of disease more than 12 months, group B: course of disease more than 36 months, group C, course of disease more than 60 months; 12 normal specimens of non-chronic tonsillitis were collected in the immunological laboratory as control group. There were 7 males and 5 females aged 755 years. Urinary microalbuminuria was determined by rate scattering turbidimetry before and after operation. The collected data were analyzed statistically. Result 1. There was no significant difference between group A and control group (P 0.05). There was significant difference between the other groups (P < 0.05). 2. The levels of urinary microalbumin in group A, group B and group C were significantly higher than those in group A, B and C before and after operation (P < 0.05). 3. The content of urinary microalbumin in patients with chronic tonsillitis was non-normal distribution. The rank correlation analysis of rs=0.659,P0.001, between the course of disease and the content of urinary microalbumin in patients with chronic tonsillitis The results showed that the course of chronic tonsillitis was correlated with the content of urinary albumin in patients with chronic tonsillitis. Conclusion 1. The urinary microalbumin excretion of chronic tonsillitis patients whose course of disease is over 60 months exceeds the normal range and can be complicated with early renal damage. 2. Urinary microalbumin as a diagnostic index for early renal damage in chronic tonsillitis has a certain clinical guiding value, and its sensitivity increases with the progression of the disease. 3. After tonsillectomy, urinary microalbuminuria decreased significantly in patients with chronic tonsillitis, and tonsillectomy was effective in the prevention and treatment of early renal damage.
