【摘要】：目的探討超聲乳化術患者術后早期應用硅水凝膠角膜接觸鏡的臨床效果。方法選擇年齡相關性白內障患者常規(guī)行3.0 mm透明角膜隧道切口超聲乳化術,術畢時隨機分為2組:戴鏡組(106例145眼)和敷料組(110例159眼)。戴鏡組術畢時予硅水凝膠角膜接觸鏡置入結膜囊,術后1 h立即開始予抗炎眼液和人工淚液滴眼(1次·h-1至睡前),并于術后1 d取出;敷料組術畢時予常規(guī)敷料包眼。觀察術前及術后1 d、1周、1個月的裸眼視力、最佳矯正視力、淚膜破裂時間(break-up time,BUT)、基礎淚液分泌量、主觀癥狀評分、眼前節(jié)體征(包括角膜水腫、角膜后沉著物、房水閃輝)評分以及白內障術后相關并發(fā)癥。結果戴鏡組在術后1 d的裸眼視力、最佳矯正視力分別為4.80±0.15和4.82±0.20,均高于敷料組,差異均有統(tǒng)計學意義(均為P0.05);而在術后1周、1個月2組間差異無統(tǒng)計學意義(均為P0.05)。戴鏡組在術后1 d BUT為(5.27±3.59)s,而敷料組為(3.37±2.61)s,差異有統(tǒng)計學意義(P0.05),2組術后各時間點均比術前縮短。2組間基礎淚液分泌量在術后各個時間點差異均無統(tǒng)計學意義(均為P0.05);術后1 d較術前增加,術后1周、1個月均較術前減少。主觀癥狀和眼前節(jié)體征戴鏡組在術后1 d均比敷料組輕,2組在術后各時間點均比術前加重,差異均有統(tǒng)計學意義(均為P0.05)。2組在術后均未發(fā)現(xiàn)白內障術后相關并發(fā)癥。結論超聲乳化術后早期配戴硅水凝膠角膜接觸鏡配合開放滴眼,不僅使患者在術后即刻擁有視力,還可安全、有效減輕早期眼前節(jié)炎癥,促進淚膜穩(wěn)定,緩解患者的不適感。
[Abstract]:Objective to investigate the clinical effect of early application of silicone hydrogel contact lens in patients with ultrasonic emulsion. Methods 3. 0 mm transparent corneal tunnel incision phacoemulsification was performed in patients with age-related cataract. At the end of the operation, they were randomly divided into two groups: the group of wearing lens (106 cases, 145 eyes) and the group of dressing (110 cases, 159 eyes). Silicone hydrogel contact lens was placed into the conjunctival sac in the wearing group, and the anti-inflammatory eye drops and artificial tear drops (once h-1 to before bedtime) were given immediately after 1 hour, and were removed on the first day after operation, while the routine dressing was used in the dressing group at 1 hour after operation. The uncorrected visual acuity, best corrected visual acuity (BCVA), tear film rupture time (break-up time,BUT), basic tear secretion, subjective symptom score, anterior segment signs (including corneal edema) were observed before and 1 day after operation. Results the uncorrected visual acuity of the wearing group was 4.80 鹵0.15 and 4.82 鹵0.20, respectively, which was significantly higher than that of the dressing group (P0.05). However, there was no significant difference between the two groups at 1 week and 1 month after operation (P0.05). The BUT was (5.27 鹵3.59) s in the mirror wearing group and (3.37 鹵2.61) s in the dressing group. The difference was significant (P0.05). The amount of basal tear secretion in both groups was shorter than that before operation. There was no significant difference between the two groups at each time point (P0.05). 1 day after operation increased compared with preoperative, 1 week after operation, 1 month decreased compared with preoperative. The subjective symptoms and anterior segment signs were lighter in the group of wearing mirror than that in the dressing group on the 1st day after operation, and were more serious in the two groups at each time point after operation than before operation. The difference was statistically significant (P0.05). No postoperative complications were found in both groups. Conclusion early wear of silicone hydrogel contact lens and open eye drop after phacoemulsification not only can make the patients have visual acuity immediately after phacoemulsification, but also can effectively reduce the inflammation of early anterior segment, promote the stability of tear film and relieve the discomfort of the patients.
