【摘要】： 目的：通過對111例不同聽力曲線類型突聾伴主觀性耳鳴患者使用金納多聯(lián)合激素治療后對耳鳴療效、聽力療效、綜合療效三方面的研究,評價此類治療方案在不同聽力曲線類型耳鳴患者中的應(yīng)用價值。為今后臨床使用此類藥物治療篩選合適的病例,這也是為廣大耳鳴患者個性化治療方案提供參考依據(jù)。 方法：研究2008年8月～2009年10月在我科住院治療的以主觀性耳鳴及聽力下降為主訴的突聾患者111例。按照聽力曲線類型分為3個研究組：下降型組42例,上升型組37例,平坦型組32例。所有病例均使用金納多聯(lián)合地塞米松激素治療,平均治療(11.7±3.0)天。 結(jié)果：經(jīng)卡方檢驗表明,3個研究組間,耳鳴療效x2=19.713、聽力療效x2=44.265、綜合療效x2=14.956,P值均0.05,差異有統(tǒng)計學(xué)意義,說明3組不同聽力曲線類型的耳鳴患者治療效果不同。上升型組在耳鳴療效、聽力療效及綜合療效三方面評價結(jié)果提示對金納多聯(lián)合激素治療較為敏感。 結(jié)論：對上升型聽力曲線的耳鳴患者推薦使用金納多聯(lián)合激素治療,而下降型聽力曲線和平坦型聽力曲線的耳鳴患者使用金納多聯(lián)合激素治療的療效有限。
[Abstract]:Objective: to study the curative effect, hearing effect and comprehensive curative effect on tinnitus in 111 patients with sudden hearing loss and subjective tinnitus. To evaluate the value of this treatment regimen in patients with tinnitus with different hearing curve types. It provides a reference for individualized treatment of tinnitus patients. Methods: 111 cases of sudden deafness with subjective tinnitus and hearing loss in our department from August 2008 to October 2009 were studied. According to the type of hearing curve, they were divided into three groups: descending group (n = 42), ascending type group (n = 37) and flat type group (n = 32). All patients were treated with Ginaton combined with dexamethasone with an average of (11.7 鹵3.0) days. Results: the chi-square test showed that the tinnitus curative effect x219.713, the hearing curative effect x24.265, the comprehensive curative effect x24.956 P value were 0.05, the difference was statistically significant, which indicated that the three groups of tinnitus patients with different hearing curve types had different therapeutic effects. The evaluation of tinnitus effect, hearing effect and comprehensive curative effect in ascending type group indicated that it was more sensitive to the combination of Ginaton and hormone therapy. Conclusion: it is recommended to treat tinnitus patients with ascending hearing curve by combination of kinnardo and hormone, while those with reduced hearing curve and flat hearing curve have limited curative effect.
