【摘要】：目的系統(tǒng)評價白內(nèi)障囊外摘除術(shù)與超聲乳化吸出術(shù)對角膜內(nèi)皮細(xì)胞及角膜中央厚度的影響。方法按Cochrane系統(tǒng)評價方法,計算機檢索MEDLINE (1966～2010.11). EMbase (1980-2010.11)、Cochrane圖書館(2008年3期)和中國生物醫(yī)學(xué)文獻(xiàn)數(shù)據(jù)庫(CBM,1979～2010.11),并手工檢索相關(guān)會議文獻(xiàn)。文獻(xiàn)語種限制為中英文。收集所有相關(guān)隨機對照試驗,采用RevMan 5.0軟件進(jìn)行Meta分析。結(jié)果共納入4篇隨機對照試驗,包括704例患者(共704只患眼),隨訪1～12個月。Meta分析結(jié)果顯示：白內(nèi)障囊外摘除術(shù)與白內(nèi)障超聲乳化吸出術(shù)在術(shù)后角膜內(nèi)皮細(xì)胞計數(shù)方面[WMD=55.26,95%CI(0.79,109.73)],P=0.05,及在術(shù)后角膜中央厚度方面[WMD=28.21,95%CI(12.29,44.13)],P=0.0005,有差異。結(jié)論與白內(nèi)障囊外摘除術(shù)相比,白內(nèi)障超聲乳化吸出術(shù)術(shù)后角膜內(nèi)皮細(xì)胞計數(shù)減少更多,角膜中央厚度增加更多。因本系統(tǒng)評價納入研究病例數(shù)較少,上述結(jié)論需更多設(shè)計嚴(yán)謹(jǐn)大樣本隨機對照試驗加以驗證。
[Abstract]:Objective to evaluate the effects of extracapsular cataract extraction and phacoemulsification on corneal endothelium and central corneal thickness. Methods according to the evaluation method of Cochrane system, MEDLINE (1966 / 2010.11) was searched by computer. EMbase (1980-2010.11) includes the Cochrane Library (3 issues 2008) and the Chinese Biomedical Literature Database (CBM,1979~2010.11). The language of the literature is restricted to Chinese and English. All the related randomized controlled trials were collected and Meta analysis was carried out with RevMan 5.0 software. Results A total of 4 randomized controlled trials were conducted. In 704 patients (704 eyes), the results of meta-analysis showed that extracapsular cataract extraction and phacoemulsification [WMD=55.26,95%CI _ (0.79109.73)] P0. 05, and corneal central thickness after cataract extraction and phacoemulsification. The degree of WMD=28.21,95%CI (12.29 ~ 44.13) was 0.0005. Conclusion compared with extracapsular cataract extraction, the number of corneal endothelium and the central thickness of cornea after phacoemulsification were decreased more and more. Due to the small number of cases included in this systematic evaluation, the above conclusions need to be verified by more well-designed randomized controlled trials with large samples.
