【摘要】：目的： 通過(guò)回顧性病例總結(jié),觀(guān)察探討中醫(yī)綜合療法對(duì)眼壓控制后的青光眼的視神經(jīng)保護(hù)作用,分析中醫(yī)證型規(guī)律。 方法： 以中國(guó)中醫(yī)科學(xué)院眼科醫(yī)院1997年-2010年門(mén)診及住院的原發(fā)性開(kāi)角型青光眼及原發(fā)性慢性閉角型青光眼患者為研究對(duì)象,總結(jié)病例共83例(164眼),其中中藥治療觀(guān)察組病例49例(97眼)。對(duì)照組病例34例(67眼)。對(duì)比中藥治療觀(guān)察組與對(duì)照組治療前后視力及視野的改變,總結(jié)中藥治療觀(guān)察組的證型分類(lèi)及各證型之間治療前后視力及視野的改變。 結(jié)果： 1.視野變化。 中藥治療觀(guān)察組中好轉(zhuǎn)18眼,穩(wěn)定75眼,惡化4眼。對(duì)照組好轉(zhuǎn)3眼,穩(wěn)定51眼,惡化13眼。2組差異具有顯著性意義(P0.01)。 觀(guān)察組中肝腎不足病例29例57眼,好轉(zhuǎn)10眼,穩(wěn)定44眼,惡化3眼,氣虛血瘀病例15例30眼,好轉(zhuǎn)6眼,穩(wěn)定23眼,惡化1眼,肝郁氣滯病例5例10眼,好轉(zhuǎn)2眼,穩(wěn)定8眼,惡化0眼。 2.視力變化： 中藥治療觀(guān)察組中視力提高≥2行10眼,視力下降≥2行3眼,介于其間84眼。對(duì)照組視力提高≥2行6眼,視力下降≥2行6眼,介于其間55眼。2組差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。 觀(guān)察組中肝腎不足病例29例57眼,好轉(zhuǎn)7眼,穩(wěn)定49眼,惡化1眼,氣虛血瘀病例15例30眼,好轉(zhuǎn)3眼,穩(wěn)定27眼,惡化0眼,肝郁氣滯病例5例10眼,好轉(zhuǎn)0眼,穩(wěn)定8眼,惡化2眼。 結(jié)論： 中醫(yī)辨證治療對(duì)眼壓控制的青光眼患者的視野擴(kuò)大有顯著意義,具有保護(hù)視神經(jīng)的功能。
[Abstract]:Objective: to investigate the protective effect of TCM combined therapy on the optic nerve after intraocular pressure control (IOP), and to analyze the pattern of TCM syndrome. Methods: primary open angle glaucoma and primary chronic angle closure glaucoma were studied in Ophthalmology Hospital of Chinese Academy of traditional Chinese Medicine from 1997 to 2010. A total of 83 cases (164 eyes) were summarized and 49 cases (97 eyes) were treated with traditional Chinese medicine. There were 34 cases (67 eyes) in the control group. The changes of visual acuity and visual field before and after treatment were compared between the observation group and the control group, and the classification of syndromes and the changes of vision and visual field before and after treatment were summarized. Results: 1. A change in vision. In the observation group, 18 eyes were improved, 75 eyes were stable and 4 eyes were worse. In the control group, 3 eyes improved, 51 eyes stabilized and 13 eyes deteriorated, the difference was significant (P0.01). In the observation group, there were 29 cases (57 eyes) with deficiency of liver and kidney, 10 eyes with improvement, 44 eyes with stability, 3 eyes with deterioration, 15 cases with Qi deficiency and 30 eyes with blood stasis, 6 eyes with improvement, 23 eyes with stability, 1 eye with deterioration, 5 cases with liver stagnation and 10 eyes with Qi stagnation, 2 eyes with improvement and 8 eyes with stability. Deterioration: 0 eyes. 2. The change of visual acuity: in the observation group, the visual acuity was improved more than 2 lines in 10 eyes, and decreased in 3 eyes in more than 2 lines, which was between 84 eyes.
