非球面晶體與球面晶體植入術(shù)后視覺(jué)質(zhì)量的對(duì)比研究

【摘要】: 目的對(duì)比研究白內(nèi)障超聲乳化聯(lián)合人工晶狀體植入手術(shù)后非球面晶狀體與球面晶狀體眼的視覺(jué)質(zhì)量。 方法將我院2009年4月至2010年4月擬行超聲乳化白內(nèi)障吸除聯(lián)合人工晶狀體植入術(shù)的年齡相關(guān)性白內(nèi)障患者106例隨機(jī)分為2大組:非球面晶狀體組與球面晶狀體組。其中非球面晶狀體組隨機(jī)分別植入球差值-0.18μm非球面人工晶狀體(HOYA60AD),球差值-0.15~-0.18μm非球面人工晶狀體(STAAR KS-3AI),零球差非球面人工晶狀體(Rayner920H或Rayner970C),非恒定球差非球面人工晶狀體(Zeiss ZO);傳統(tǒng)球面人工晶狀體組隨機(jī)分別植入Rayner570C,XLSTABI-SKY,Hexavision HQ-201人工晶狀體。術(shù)后1個(gè)月對(duì)患者進(jìn)行裂隙燈下眼前段和眼底檢查、電腦驗(yàn)光及最佳矯正視力檢查,對(duì)除外眼部并發(fā)癥及裸眼視力或最佳矯正視力≥0.8患者行明視和暗視眩光及無(wú)眩光條件下的對(duì)比敏感度、6mm瞳孔直徑時(shí)眼的總體像差檢測(cè)和視功能與生存質(zhì)量問(wèn)卷調(diào)查。 結(jié)果術(shù)后1個(gè)月,7組人工晶狀體患者最佳矯正遠(yuǎn)視力差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);明視、明視眩光中低空間頻率和暗視高空間頻率下4種非球面晶狀體組對(duì)比敏感度值總體高于球面晶狀體組。四種非球面晶體間比較:在明視及暗視有眩光或無(wú)眩光的各空間頻率下非恒定球差和球差值-0.15~-0.18μm非球面人工晶狀體組相較球差值-0.18μm非球面人工晶狀體組及零球差非球面人工晶狀體組對(duì)比敏感度值高,其中非恒定球差非球面人工晶狀體組在明暗視中、低空間頻率高,球差值-0.15~-0.18μm非球面人工晶狀體組在明視中高空間頻率及暗視低空間頻率高。三組球面晶狀體組比較:明視各空間頻率和暗視眩光低空間頻率時(shí)XLSTABI-SKY對(duì)比敏感度值高,暗視眩光中高空間頻率下Rayner570C相對(duì)較高,但與XLSTABI-SKY差異無(wú)統(tǒng)計(jì)學(xué)意義。Zeiss非球面晶體在明視和暗視有無(wú)眩光時(shí)中高空間頻率下相較Zeiss球面晶體對(duì)比敏感度值高。而Rayner非球面晶體在明視有無(wú)眩光的中低空間頻率下對(duì)比敏感度值高于Rayner球面晶體,但在明視高空間頻率及暗視各空間頻率下Rayner球面晶體對(duì)比敏感度值高于Rayner非球面晶體。術(shù)后1個(gè)月,瞳孔直徑6mm情況下非球面人工晶狀體組的總眼球差為(0.17士0.25)μm,球面人工晶狀體組為(0.31士0.24)μm,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。VF/QOL問(wèn)卷調(diào)查顯示7組人工晶狀體眼之間的視功能與生存質(zhì)量差異有統(tǒng)計(jì)學(xué)意義(P0.05 )。 結(jié)論超聲乳化白內(nèi)障吸除聯(lián)合人工晶狀體植入術(shù)后,非球面人工晶狀體相較球面晶狀體可以降低總眼球差,相對(duì)提高視覺(jué)對(duì)比度;其中非恒定球差非球面人工晶狀體和球差值-0.15~-0.18μm非球面人工晶狀體與本研究中其他非球面晶狀體相比,能夠相對(duì)明顯的提高白內(nèi)障患者術(shù)后明暗對(duì)比敏感度,改善患者視覺(jué)質(zhì)量。
[Abstract]:Objective to compare the visual quality of aspherical lens and spherical lens after phacoemulsification combined with intraocular lens implantation. Methods from April 2009 to April 2010, 106 patients with age-related cataract undergoing phacoemulsification and intraocular lens implantation were randomly divided into two groups: aspheric lens group and spherical lens group. The aspheric lens group was randomly implanted with spherical difference (-0.18 渭 m), spherical difference (-0.15 渭 m) -0.18 渭 m aspheric intraocular lens (STAAR KS-3AI), zero spherical aberration aspheric intraocular lens (Rayner920H or Rayner970C) and unsteady spherical aberration aspheric intraocular lens (Rayner920H or Rayner970C). The conventional spherical intraocular lens (Zeiss ZO);) group was randomly implanted with Rayner 570 Con XLSTABI-SKYY Hexavision HQ-201 intraocular lens. One month after operation, the patients were examined with slit lamp under anterior segment and fundus, computer optometry and best corrected visual acuity (BCVA).
